While a representative initially stated pneumonia as the cause of death, his death certificate, obtained by Us Weekly, lists a subarachnoid hemorrhage and blunt head trauma as the immediate causes. The records indicate that Mackie suffered a fall and a subsequent head injury at the home of his longtime design director, Joseph McFate. He passed away three days later at Desert Regional Medical Center in Palm Springs. A representative clarified that Mackie was staying in the guest house at McFate's home, and a healthcare professional was present when he fell. The representative also noted that Mackie was suffering from pneumonia both before and after his hospitalization. Mackie was cremated on September 17, and his remains were scattered off the coast of San Diego County, California. His death was announced on social media on the day he died, with a post highlighting his fulfillment of his dream as a fashion and costume designer.
