HRSA Revives 340B Rebate Pilot Program Amidst Congressional Opposition
The Health Resources and Services Administration (HRSA) has revived its 340B Rebate Model Pilot Program, announcing a revised version on July 31, 2026, and formalizing it in the Federal Register on August 3, 2026. This marks HRSA's second attempt in just over a year to transition a subset of 340B drugs from upfront discounts to retrospective manufacturer rebates. Under the revised pilot, participating covered entities will purchase affected drugs at wholesale acquisition cost (WAC) and then submit claims to manufacturers for a rebate equal to the difference between WAC and the 340B ceiling price. Manufacturers must submit rebate plans to HRSA by August 24, 2026, with approvals expected by September 24, 2026, and the pilot launching on January 1, 2027, for at least one year.
