Study on Delayed Cord Clamping in Preterm Twins Shows Potential Cardiovascular Benefits
A recent prospective observational study conducted at CHU Sainte Justine in Montreal has examined the effects of delayed cord clamping (DCC) versus early cord clamping (ECC) in preterm twins. The study involved preterm neonates between 24 and 36 weeks of gestational age, excluding those with congenital malformations, twin-to-twin transfusion syndrome, or those requiring ECC for resuscitation. The research focused on echocardiographic metrics to assess systemic blood flow. Out of 63 twins studied, 47 underwent DCC while 16 experienced ECC. The findings indicated that while superior vena cava blood flow was similar between the two groups, left ventricular output was higher in the ECC group at 24-48 hours. However, the DCC group showed improved right ventricular function and cardiovascular stability, as evidenced by higher TAPSE Z-scores and systolic velocity of the right ventricle lateral wall.
