SocialRx Partners with NYC Health Center to Prescribe Arts for Chronic Illness
SocialRx, a social prescribing platform, has partnered with Urban Health Plan, a federally qualified health center in New York City, to offer arts and cultural experiences as part of treatment for chronically ill patients. This initiative targets patients aged 18 to 24 with obesity and depression, and those aged 55 to 70 with hypertension and depression. The program aims to improve mental health and reduce social isolation by connecting patients to community-based activities. Patients receive a 'prescription' for arts and culture, and a care navigator helps them create a personalized well-being plan. The program also includes follow-ups to assess the impact of these activities on patients' health.
