The facility believes the seed, pre-loaded into a needle, was inadvertently discarded in a sharps container while still within its lead shielding. Corrective actions include requiring two individuals to concurrently update and account for I-125 seeds. In Texas, two Troxler moisture density gauges, containing cesium-137 and americium-241/beryllium sources, were lost in transport by a common carrier. Quest Academy Expands Deployment of ZeroEyes Gun Detection Technology

Quest Academy Charter School in West Haven, Utah, has expanded its implementation of ZeroEyes' gun detection and threat intelligence software. This expansion follows a successful initial year of deployment. ZeroEyes, founded by Navy SEALs and technologists, provides AI-based gun detection that integrates with existing digital security cameras. The technology is designed to identify visibly brandished weapons, such as guns and knives, and alert local staff and law enforcement with images and precise location data, often within seconds. The system is monitored 24/7 by the ZeroEyes Operations Center, staffed by U.S. military and law enforcement veterans. The initial deployment at Quest Academy was supported by the Utah State Board of Education’s School Safety and Support Grant Program, and the school has chosen to increase coverage across its campus to enhance situational awareness and security for its approximately 1,000 students and 100 faculty and staff.
