A chemical leak at the Catalyst Refiners plant in Institute, West Virginia, has resulted in the deaths of two people and sent 19 others to the hospital. The incident occurred during a shutdown process at the facility, involving a chemical reaction between nitric acid and another substance, which produced toxic hydrogen sulfide gas. The leak required a large-scale decontamination operation, and a shelter-in-place order was issued for the surrounding area. The plant, part of Ames Goldsmith Corp., is involved in silver recovery from chemical processes. The Occupational Safety and Health Administration (OSHA) has initiated an investigation into the incident.
