CMS Launches ACCESS Model to Integrate Technology in Chronic Disease Management
The Centers for Medicare and Medicaid Services (CMS) has selected 150 digital health companies and providers to participate in the Advancing Chronic Care with Effective Scalable Solutions (ACCESS) Model. This 10-year initiative aims to integrate technology into chronic disease management, offering stable payments for tech solutions addressing conditions like diabetes, hypertension, and depression. The model will support telehealth, wearables, and wellness apps, with payments aligned to clinical outcomes. The program, starting July 5, 2026, seeks to enhance care options for Medicare beneficiaries, with commercial payers also aligning with the model's payment approach.
