Uneven Care Landscape
India's healthcare infrastructure grapples with significant inconsistencies, a point underscored by a recent multi-state stakeholder consultation in Ahmedabad.
This gathering focused on recommendations from the Lancet Commission, which advocates for a citizen-centric health system designed to tackle contemporary health issues and build an integrated national framework. A key concern voiced by Mirai Chatterjee, a director at SEWA and a commissioner on the Lancet panel, revolves around the persistent 'uneven quality of care, fragmented delivery and poor governance' plaguing the nation's health services. This is further exacerbated by a noticeable rise in mental health concerns and deficiencies in the effective implementation of health programs at the grassroots level. The report emphasizes that its foundation lies in understanding and responding to the needs and aspirations of citizens, rather than viewing health solely through a systemic lens. However, the reality on the ground reveals significant deficits, especially concerning mental health services at the primary healthcare tier and a scarcity of accessible local counselors, hindering comprehensive well-being.
Integrated System Vision
The pursuit of universal health coverage in India hinges on the establishment of a robust, publicly funded, and publicly delivered integrated healthcare system. Vikram Patel, co-chair of the Lancet Commission and a professor at Harvard Medical School, strongly emphasized this point during discussions that followed the report's release. The consultation, organized by SEWA’s Lok Swasthya SEWA initiative, brought together participants from Gujarat, Rajasthan, and Madhya Pradesh, fostering a dialogue on implementing the commission's findings. The core idea is to move beyond fragmented services and create a seamless continuum of care, where every citizen can access essential health services without facing financial or geographical barriers. This integrated model aims to enhance efficiency, improve accountability, and ensure that public health initiatives reach the last mile effectively, thereby strengthening the overall health ecosystem and moving closer to the goal of providing quality healthcare for every individual across the country.
