What's Happening?
Immunome, Inc., a clinical-stage oncology company, announced that the European Medicines Agency (EMA) has validated its Marketing Authorization Application (MAA) for varegacestat, an investigational oral gamma secretase inhibitor (GSI), for the treatment
of adults with desmoid tumors. This validation confirms the completeness of the application, allowing the EMA to begin its review process. The MAA is supported by positive results from the Phase 3 RINGSIDE trial, which demonstrated statistically significant improvements in progression-free survival and objective response rates compared to placebo. Varegacestat also showed significant improvement in pain intensity and a median best change in tumor volume of -83%. The drug was generally well-tolerated, with most adverse events being mild to moderate. This EMA validation follows the U.S. Food and Drug Administration (FDA) acceptance of Immunome's New Drug Application (NDA) for varegacestat in July 2026, with a Prescription Drug User Fee Act (PDUFA) target action date of April 28, 2027.
Why It's Important?
The EMA validation and ongoing FDA review of varegacestat are critical steps toward providing a much-needed treatment option for adults with desmoid tumors in the U.S. and globally. Desmoid tumors are aggressive, non-metastatic soft tissue tumors that can cause debilitating pain, deformity, and life-threatening organ damage, with no approved disease-modifying therapies currently available. The positive Phase 3 RINGSIDE trial results, showing significant reductions in disease progression and tumor volume, highlight varegacestat's potential to address this high clinical burden and improve patients' quality of life. For U.S. patients, the potential approval of varegacestat would offer a new oral, once-daily treatment, representing a significant advancement in managing this rare disease and potentially preventing permanent disability.
What's Next?
Immunome is awaiting the FDA's decision on the New Drug Application for varegacestat, with a target action date of April 28, 2027. Concurrently, the EMA will proceed with its review of the Marketing Authorization Application. Successful approval from both regulatory bodies would pave the way for varegacestat to become available to patients in the U.S. and Europe. Immunome continues to advance its pipeline of targeted oncology therapeutics, including other antibody-drug conjugates and a FAP-targeted radiotherapy, indicating a broader commitment to addressing unmet needs in cancer treatment. The company's focus will now shift towards potential commercialization strategies and ensuring patient access if regulatory approvals are granted.
Beyond the Headlines
The development of varegacestat for desmoid tumors underscores the growing focus on rare diseases, where patient populations are small but the medical need is often profound. The success of targeted therapies for such conditions can provide significant relief to affected individuals and their families. However, the economic viability of developing drugs for rare diseases often relies on orphan drug designations and expedited regulatory pathways, which are crucial for incentivizing pharmaceutical companies. The global regulatory efforts by Immunome, seeking approvals in both the U.S. and Europe, highlight the interconnectedness of international drug development and the importance of harmonized regulatory processes to bring treatments to patients worldwide. This also raises questions about equitable access and pricing strategies across different healthcare systems.
