This diet is most frequently discussed in relation to autism, with proponents often claiming it can alleviate or even cure autism symptoms. However, these claims largely rely on anecdotal evidence or non-scientific sources, as rigorous

Despite the enthusiasm from some advocates, there is no conclusive scientific support for the efficacy of the GFCF diet in treating autism. Some parents report perceived improvements in their children's autism-related symptoms and behavioral changes in the initial days of implementing exclusion diets. Such accounts have appeared in popular media, with a few parents even claiming their children were cured. For instance, a family reported in a French publication that a diet excluding milk, gluten, and sugars resolved their child's recurring ear infections. However, these individual reports, while emotionally compelling, do not constitute scientific evidence and are regarded as unsubstantiated by the broader medical and scientific community.
Hypotheses and Criticisms
One hypothesis, proposed by Reichelt, suggested that long-term exposure to opiate peptides derived from gluten and casein might affect brain maturation and contribute to social awkwardness and isolation in individuals with autism. Based on this, Reichelt and others proposed the GFCF diet to minimize the buildup of these peptides. While Reichelt has published trials and reviews concluding the diet's effectiveness, these findings have not been widely replicated or supported by independent, rigorous research.
The medical community maintains a consensus that the claims of curing autism through dietary interventions like the GFCF diet are not supported by scientific evidence. The focus on such diets often diverts attention from evidence-based therapies and can introduce unnecessary risks. The perceived harmlessness of dietary interventions, particularly by parents, often leads to an underestimation of their potential negative consequences, especially during the initial stages of implementation.
Documented Side Effects and Social Impact
Several studies have highlighted the significant risks associated with long-term adherence to restrictive diets such as GFCF regimens. These include micronutrient deficiencies, which necessitate ongoing monitoring to ensure adequate nutritional intake. Research has identified risks of malnutrition, and one study reported reduced cortical bone thickness as a potential side effect in a child following prolonged adherence to a casein-free diet. Cases of severe malnutrition have been documented in non-autistic individuals who followed these diets without consistent medical supervision.
Beyond the physical health risks, the GFCF diet can impose substantial economic, social, and psychological burdens on families. The financial impact is compounded by the need to purchase specialized foods and dietary supplements to prevent deficiencies. Social isolation is another significant concern, particularly for autistic children, as excluding specific foods can make social situations like birthday parties, family meals, or school cafeterias challenging. This outcome contradicts the primary objective of many autism interventions, which prioritize social integration. Gastroenterologists also point to the potential psychological effects of depriving children of typical pleasures, such as sweets, in an attempt to manage autism, and the diet may worsen existing eating challenges for autistic children with high food selectivity by removing preferred foods.
