Physical exercise is a powerful tool for enhancing the function of numerous bodily systems, contributing significantly to overall health and disease prevention. Its effects are far-reaching, influencing everything from the strength of the heart to the resilience of the immune system and even the quality of sleep. Cardiovascular Benefits of Exercise
The beneficial effect of exercise on the cardiovascular system is extensively documented. There is a direct correlation between physical inactivity and cardiovascular disease, with inactivity being an independent risk factor for the development of coronary artery disease. Low levels of physical exercise significantly increase the risk of cardiovascular disease mortality. Children who engage in physical exercise experience greater loss of body fat and increased cardiovascular fitness, setting a foundation for lifelong health. Studies have also shown that while academic stress in youth can increase the risk of cardiovascular disease in later years, these risks can be substantially mitigated with regular physical exercise.
There is a dose-response relationship between the amount of exercise performed, specifically an energy expenditure of approximately 700–2000 kcal per week, and all-cause mortality and cardiovascular disease mortality in middle-aged and elderly men. The most significant potential for reduced mortality is observed in sedentary individuals who transition to being moderately active. For women, particularly aging women, regular exercise leads to healthier cardiovascular profiles, which is crucial given that heart disease is a leading cause of death in this demographic. Moderate-intensity activity, typically 40–60% of maximal oxygen uptake depending on age, yields the most beneficial effects on cardiovascular disease mortality. Survivors of myocardial infarction who adopt regular exercise into their lifestyle demonstrate higher survival rates. Sedentary individuals face the highest risk for mortality from cardiovascular and all other causes. The American Heart Association emphasizes that exercise reduces the risk of cardiovascular diseases, including heart attack and stroke. Furthermore, increased physical exercise has been suggested to decrease healthcare costs, improve job attendance rates, and enhance the effort women put into their jobs.
Exercise and the Immune System
While hundreds of studies have explored the relationship between physical exercise and the immune system, direct evidence linking it to illness is still somewhat limited. However, epidemiological evidence suggests that moderate exercise has a beneficial effect on the human immune system, often modeled as a J-curve. Moderate exercise has been associated with a 29% decreased incidence of upper respiratory tract infections (URTI). Conversely, studies on marathon runners indicate that prolonged, high-intensity exercise may be linked to an increased risk of infection, though another study did not find this effect. Immune cell functions can be impaired following acute sessions of prolonged, high-intensity exercise, and some research suggests athletes might be at a higher risk for infections. Strenuous stress over long durations, such as marathon training, can suppress the immune system by reducing lymphocyte concentration. Despite this, the immune systems of athletes and non-athletes are generally similar, though athletes may have a slightly elevated natural killer cell count and cytolytic action, which are unlikely to be clinically significant. Vitamin C supplementation has been associated with a lower incidence of URTIs in marathon runners. Biomarkers of inflammation, such as C-reactive protein, which are linked to chronic diseases, are reduced in active individuals compared to sedentary ones, suggesting that exercise's positive effects may stem from its anti-inflammatory properties. In individuals with heart disease, exercise interventions lower blood levels of fibrinogen and C-reactive protein, both important cardiovascular risk markers. The temporary depression in the immune system following acute bouts of exercise might be one of the mechanisms behind this anti-inflammatory effect.
Exercise, Sleep, and Libido
Preliminary evidence from a 2012 review indicated that physical training for up to four months could enhance sleep quality in adults over 40 years of age. A 2010 review suggested that exercise generally improved sleep for most people and might help with insomnia, though more detailed conclusions about the relationship between exercise and sleep require further evidence. A 2018 systematic review and meta-analysis further suggested that exercise can improve sleep quality in individuals with insomnia. Beyond sleep, a 2013 study found that exercising improved sexual arousal problems related to antidepressant use, highlighting another less commonly discussed benefit of physical activity.
