The Downhill Dilemma
Running downhill feels like it should be easy, but it places immense stress on your body. To control your speed and prevent a fall, your quadriceps muscles work like brakes. This braking action is an 'eccentric' muscle contraction, where the muscle is firing
while it lengthens. This places a significant load on the patellar tendon, the robust cord connecting your kneecap to your shinbone. Over time, especially with the repetitive impact of running, this load can lead to micro-damage, pain, and a condition known as patellar tendinopathy. It’s not just inflammation; it's often a degenerative process where the tendon's strong, organized collagen fibers become disorganized and weak.
Meet Eccentric Training
Eccentric training isn't complicated. It simply means focusing on the part of an exercise where the muscle lengthens under tension. For example, think of a squat. The concentric part is standing up; the eccentric part is the slow, controlled descent into the squat. By isolating and emphasizing this lowering phase, you are performing an eccentric exercise. The goal is to apply a specific, controlled stress to the tendon, encouraging it to adapt and grow stronger without the explosive force of concentric or plyometric movements.
The Science of Tendon Repair
So why does this work? Think of your tendon as a sponge. To heal, it needs to flush out waste and bring in new, nutrient-rich fluid. The controlled stress from slow eccentric exercises essentially 'squeezes' the tendon, stimulating a cellular process called mechanotransduction. This process signals your tendon cells to produce new, high-quality Type I collagen, the building block of healthy tendons. Over time, this helps realign the disorganized fibers, effectively remodeling the tendon to be stronger, stiffer, and better able to handle the loads of running. This increased load capacity is precisely what protects it from future wear and tear.
Essential Eccentric Exercises
One of the most effective exercises for patellar tendinopathy is the single-leg eccentric decline squat. This is often done on a slant board or a small ramp angled at about 20 degrees to specifically target the patellar tendon. To perform it, stand on your affected leg on the board, facing down the slope. Slowly, over three to five seconds, squat down as far as you can without sharp pain. Then, place your other foot down and use both legs (or primarily your good leg) to return to the starting position. The key is to load the tendon only on the way down. Another great exercise is the simple eccentric step-down. Stand on a step or box and slowly lower your opposite heel toward the floor, maintaining control throughout the movement.
Integrating Training Safely
Integrating eccentric work into your routine should be done thoughtfully. It's not about adding more fatigue; it's about targeted strengthening. Start with two to three sessions per week on non-running days or after a light run. A common protocol involves 3 sets of 15 repetitions, performed slowly. It’s normal to feel some discomfort in the tendon during the exercise, but you should not feel sharp, debilitating pain. This isn't a quick fix, but a long-term strategy. Studies have shown that consistent eccentric training over several weeks can significantly reduce pain and improve function, allowing runners to get back to the hills they love with more confidence and less pain.
