Why Hills Punish Your Knees
Every runner knows the feeling: cresting a hill is exhilarating, but the descent can be brutal. When you run downhill, your quadriceps muscles work like brakes to control your speed and absorb impact. This braking action involves an 'eccentric' muscle contraction,
where the muscle lengthens under tension. This places a massive load on the patellar tendon, the connective tissue linking your kneecap to your shinbone. Over time, especially with the repetitive impact of a long road race, this can lead to micro-tears and degeneration within the tendon, a condition known as patellar tendinopathy, or 'runner's knee'. This is why your knees often feel much worse after a hilly race compared to a flat one.
Eccentric vs. Concentric: A Quick Primer
To understand the solution, we first need to grasp two types of muscle work. A 'concentric' contraction is when a muscle shortens to create movement, like when you push up from a squat. An 'eccentric' contraction is when the muscle lengthens while under load, like the controlled lowering phase of that same squat. Think of it as the difference between lifting a box (concentric) and placing it down gently (eccentric). Downhill running is an almost continuous eccentric load on your quads. The problem isn't the eccentric load itself, but being unprepared for it.
How 'Slow-Eccentric' Heals and Hardens
This is where the magic of slow-eccentric training comes in. Research shows that performing heavy, slow eccentric movements stimulates tendon cells to produce new, healthy Type I collagen—the strong, resilient protein that gives tendons their strength. This process, known as mechanotransduction, essentially squeezes the old, disorganized tissue and encourages the growth of new, neatly aligned fibres. This not only helps heal existing tendinopathy but also builds a stronger, thicker, and more durable tendon that is better equipped to handle the specific stresses of downhill running. You are essentially teaching the tendon to become stronger and better at absorbing force.
Key Exercises to Get Started
The key is to focus on the slow, controlled lowering phase of each movement. The 'up' phase can be quicker or assisted. Aim for a lowering count of 3 to 5 seconds. Start with two sessions per week on non-running days. Here are two foundational exercises: 1. Eccentric Step-Downs: Stand on a small step or the bottom stair. With one leg, slowly lower your other heel to the ground over 3-5 seconds, keeping your knee stable and aligned over your foot. Barely touch the ground, then use your non-working leg to help you step back up. Don't push up with the working leg. Do 3 sets of 10-15 reps on each side. 2. Decline Squats: If you have access to a slant board, this is a classic. Stand on the board with your feet pointing forward. Slowly lower yourself into a squat, focusing the work on the target leg if doing it single-legged. The decline angle specifically loads the patellar tendon. Go as low as you can without significant pain, then push back up. If doing this with both legs, the slow lowering is still the most important part. Aim for 3 sets of 15 reps.
Integrating Into Your Training
Consistency is more important than intensity in the beginning. Start with just your body weight. You may feel some delayed onset muscle soreness (DOMS) initially, which is normal. As you get stronger, you can add weight using a backpack or dumbbells. Listen to your body; you might feel some discomfort in the tendon during the exercise, but it shouldn't be sharp or severe, and it should not be worse the next day. By incorporating these targeted exercises into your routine, you are not just treating pain; you are building a more robust and resilient running body, ready to take on the challenges of any hilly course with confidence.
