For years, sugar alcohols like erythritol and xylitol have been celebrated in health-conscious circles, especially among those following low-carb and keto diets. They offered the sweetness of sugar without the calories or the blood sugar spike. However,

This new research suggests a troubling link between high levels of these specific sweeteners in the blood and an increased risk for serious cardiovascular events, including heart attacks and strokes. Erythritol and Xylitol in the Spotlight
The two main sugar alcohols under scrutiny are erythritol and xylitol. Erythritol is found in many 'zero-sugar' and 'keto-friendly' products, from baked goods to beverages, often blended with other sweeteners like stevia or monk fruit. Xylitol is similarly used in sugar-free candies, gums, and even toothpaste. While both occur naturally in small amounts in fruits and vegetables, and are even produced by our own bodies, the quantities found in processed foods can be over a thousand times greater. This high level of consumption is what researchers are now examining closely.
The Link to Blood Clots and Heart Risk
The core of the concern lies in how these sweeteners appear to affect blood platelets, the tiny cells that help our blood to clot. Studies have shown that both erythritol and xylitol can make platelets more prone to clumping together. In laboratory tests and clinical interventions, consuming a drink with a typical 30-gram serving of either sweetener was shown to significantly increase the potential for blood clot formation. This heightened clotting risk is the proposed mechanism that could lead to a heart attack or stroke, particularly in individuals who may already have underlying cardiovascular risk factors like diabetes or obesity. One study found that people with the highest blood levels of xylitol were about 50% more likely to experience a cardiovascular event over a three-year period.
Understanding the Research
Researchers made this connection through a combination of methods. First, they analyzed blood samples from thousands of patients, finding that those with the highest circulating levels of erythritol or xylitol had a significantly higher risk of suffering a heart attack or stroke in the following years. To understand the 'why', they conducted lab experiments, exposing human blood and platelets to the sweeteners and observing the increased clotting activity. Finally, in human intervention trials, they gave healthy volunteers a sweetened drink and measured the immediate spike in clotting potential in their blood, confirming that consuming these sweeteners directly and immediately impacts platelet function. This multi-pronged approach strengthens the association found in the initial patient observations.
A Call for Caution, Not Panic
While these findings are serious, experts advise against panic. The research shows a strong association and a plausible mechanism, but many of the large-scale studies were observational and focused on individuals who were already at higher risk for heart disease. Still, the evidence is compelling enough that scientists are calling for further investigation into the long-term safety of these sugar substitutes. For the average consumer, the key takeaway is to be more mindful. Reading ingredient labels on 'diet' or 'keto' products is more important than ever. While completely avoiding these sweeteners may not be necessary for everyone, moderation is strongly advised until more is known about their long-term effects.
