Why Hills Challenge Your Knees
Running on varied terrain is one of the joys of the sport, but the descent can be particularly demanding on your body. When you run downhill, your quadriceps muscles work overtime to act as brakes, controlling your momentum and absorbing impact. This
places significant stress on the patellar tendon, which connects your kneecap to your shinbone. Over time, this repetitive strain can lead to irritation, inflammation, and the dreaded condition known as runner's knee or patellar tendinopathy. Unlike running on flat ground, the braking force required for descents causes muscles to lengthen while under tension—a specific type of contraction that, if not properly trained for, can overload the tendon's capacity.
What Is Slow-Eccentric Loading?
The solution lies in training your muscles to handle this specific stress. Enter slow-eccentric loading. An "eccentric" contraction occurs when a muscle lengthens under a load. Think about the lowering phase of a squat—as you descend, your quad muscles are lengthening to control your body weight. This is the eccentric part. The "slow" component is crucial. By performing this lowering movement very slowly, typically over three to five seconds, you maximize the time your muscle and tendon spend under tension. This controlled stress is the key to stimulating adaptation and building resilience without causing further irritation.
How It Rebuilds and Strengthens Tendons
Tendons are not like muscles; they have a limited blood supply and rely on mechanical forces to signal repair and growth. Slow-eccentric loading provides the perfect stimulus for this process, a concept known as mechanotransduction. The sustained tension effectively 'squeezes' the tendon cells, prompting them to produce new, high-quality collagen—the primary protein that gives tendons their strength. This process helps to reorganise disorganised or damaged collagen fibres into a stronger, more parallel alignment. The result is a thicker, stiffer, and more durable tendon that can handle the high-impact loads of downhill running without breaking down.
Key Exercises to Get Started
Integrating slow-eccentric exercises into your routine is straightforward. The focus should always be on the controlled, slow lowering phase. Here are two effective exercises: 1. Single-Leg Eccentric Step-Downs: Stand on a small step or sturdy box. Extend one leg out in front of you. Slowly, over a count of three to five seconds, bend your standing leg to lower your other heel toward the floor. Go as low as you can comfortably control without pain. Lightly tap your heel, then use both legs to push back up to the starting position. The work is all in the slow descent. 2. Slow-Decline Squats: Place your heels on a small weight plate or a decline board to elevate them slightly. This shifts more load onto the patellar tendon. With your feet shoulder-width apart, slowly lower into a squat over three to five seconds. Go to a depth that feels challenging but pain-free, then return to the start at a normal speed. You can perform this with two legs and progress to single-leg versions as you get stronger.
Putting It Into Practice Safely
Consistency is more important than intensity when starting. Aim to perform these exercises two to three times a week on non-running days to allow for adequate recovery. Start with a volume you can handle, such as 3 sets of 10-15 repetitions per exercise. A little bit of discomfort during the exercise can be acceptable, but it should not be sharp or increase significantly. Over time, you can progress by adding weight (holding a dumbbell or wearing a weighted vest), increasing the repetitions, or slowing down the eccentric phase even more. This gradual approach allows your tendons the time they need to adapt and grow stronger, turning your knees from a point of weakness into a source of power on your next hilly run.
