Running downhill feels like you're getting a gravitational assist, but it places a heavy burden on your knees. Each step requires your quadriceps muscles to act as brakes to control your speed and absorb impact. This action puts immense eccentric strain
on the patellar tendon, which connects your kneecap to your shinbone. Over time, this repetitive stress can lead to micro-damage, pain, and a condition known as patellar tendinopathy, often nicknamed "runner's knee." Unlike running on flat ground, the constant deceleration required for descents significantly magnifies the forces acting on the tendon, making it a primary site for overuse injuries.
Understanding Eccentric Contractions
To understand the solution, we first need to grasp the concept of an eccentric muscle contraction. Think of a simple bicep curl. Lifting the weight towards your shoulder is the 'concentric' phase, where the muscle shortens. Slowly lowering the weight back down is the 'eccentric' phase, where the muscle lengthens while still under tension. Life is full of eccentric movements, from walking down stairs to lowering yourself into a chair. For runners, this lengthening-under-load action is what happens in the quadriceps and patellar tendon during downhill running, and it's where the muscle-tendon unit is most vulnerable.
Why Slow and Steady Fortifies Tendons
This is where slow-eccentric loading comes in. By intentionally performing exercises that focus on a slow, controlled lowering phase (typically 3-5 seconds), you can trigger powerful adaptations within the tendon. This specific type of loading stimulates cells within the tendon to produce new, healthy collagen—the primary building block of connective tissue. It helps reorganise disorganised and damaged collagen fibres into a stronger, more resilient structure. Think of it like squeezing out a dirty sponge and allowing it to soak up clean water; the slow loading helps remodel the tissue. This process increases the tendon's stiffness and overall load capacity, making it better equipped to handle the high-impact forces of future downhill runs.
Key Exercises for Patellar Health
Incorporating slow-eccentric exercises into your routine is straightforward. The most evidence-backed exercise for patellar tendinopathy is the single-leg eccentric decline squat. Stand on a small slant board or a slight downhill slope. Balancing on one leg, slowly lower yourself into a squat over 3-5 seconds, keeping the movement controlled. At the bottom, use both legs to push back up to the starting position to minimise stress on the concentric phase. The focus is entirely on the slow, deliberate lowering. Another great option is an eccentric leg press at the gym or a simple bodyweight eccentric squat, where you focus on a slow descent before rising back up.
Integrating Eccentrics Into Your Routine
You don't need to replace your entire strength program. Aim to incorporate these exercises 2-3 times per week on non-consecutive days. Start with 3 sets of 10-15 repetitions. It's normal to feel some discomfort in the tendon area when starting, but it shouldn't be sharp or severe. The key is consistency and progressive overload. As the exercises become easier, you can add a small amount of weight, perhaps by wearing a backpack. This is not about lifting heavy; it's about the quality and control of the slow, lengthening movement.
