The Downhill Dilemma for Your Knees
Running downhill feels like you're flying, but it places immense stress on your body. Your quadriceps muscles work overtime to act as brakes, controlling your descent and preventing a tumble. This braking force is transmitted through the patellar tendon,
the strong band of tissue connecting your kneecap to your shinbone. When you run on steep routes, especially during the downhill portion, the repetitive impact and high load can lead to micro-damage in the tendon. Over time, if the stress outpaces your body's ability to repair, this can result in patellar tendinopathy, a condition commonly known as "runner's knee" or "jumper's knee". This isn't just inflammation; it's a degenerative process where the tendon's organised collagen fibres become disorganised and weaker.
Understanding Eccentric Movements
To understand the solution, we first need to understand the movement. Every exercise has two main phases: concentric and eccentric. A concentric contraction is when the muscle shortens to create movement, like when you lift a dumbbell in a bicep curl. An eccentric contraction is when the muscle lengthens under tension, like when you slowly lower that same dumbbell. Think about a simple squat: going down is the eccentric phase for your quadriceps, and pushing back up is the concentric phase. The eccentric portion is all about control and absorbing force—precisely what your quads do when you run downhill.
The 'Slow' Secret to Tendon Health
The magic happens when you deliberately slow down that eccentric phase. Slow-eccentric movements involve performing the lengthening part of an exercise in a very controlled manner, often over three to five seconds. This extended time under tension is what makes the difference. Research suggests this slow, controlled loading stimulates the tendon cells (tenocytes) to produce new, healthy Type I collagen—the strong, organised protein that gives tendons their strength. This process helps to remodel the disorganised fibres that cause tendinopathy, effectively rebuilding the tendon from the inside out and making it more robust.
Putting It Into Practice: Key Exercises
Incorporating slow-eccentric exercises into your routine doesn't have to be complicated. The goal is to load the patellar tendon in a controlled way to stimulate adaptation. You can start with these two highly effective movements.
1. Single-Leg Eccentric Step-Downs: Stand on one leg on a small step or stair. Slowly, over a count of 3-5 seconds, bend your standing knee to lower your other heel to the floor. Tap your heel gently, then use your non-exercising leg to help you push back up to the starting position. The focus is entirely on the slow lowering phase.
2. Slow Bodyweight Squats: Stand with your feet shoulder-width apart. Lower yourself down as if sitting in a chair, taking a full 3-5 seconds to reach the bottom of your squat. Pause for a moment, then return to the standing position at a normal speed. You are stronger in the eccentric phase, so focusing on this control is key.
How to Integrate Into Your Training
To get the benefits without compromising your runs, aim to perform these exercises two to three times per week on non-running days or after an easy run. Consistency is more important than intensity. Start with just your body weight, performing 3 sets of 15 repetitions. It is normal and even necessary to feel some minor discomfort in the tendon while doing the exercises, as this signals that you are loading it correctly. However, the pain should not become progressively worse. As you get stronger, you can add a light weight (holding a dumbbell) or perform the exercises on a decline board to increase the load on the patellar tendon. Listen to your body and progress gradually.
