The Challenge of the Downhill
Every runner knows the thrill of conquering a steep incline. But what goes up must come down, and it’s often the downhill portion that poses the greatest risk of injury. As you run downhill, your muscles must act as brakes to control your descent and absorb
impact. This braking action involves what’s known as an eccentric muscle contraction. Unlike a concentric contraction where the muscle shortens (like in a bicep curl), an eccentric contraction involves the muscle lengthening while under tension. Think of slowly lowering a heavy weight; your muscle is active and engaged but getting longer. This places significant stress on the connected tendons, particularly the Achilles and patellar tendons, which are common sites of injury for runners.
What is Slow-Eccentric Movement?
Slow-eccentric training isolates and exaggerates this lengthening phase of muscle contraction. It involves performing the lowering part of an exercise in a very slow, controlled manner, typically over three to four seconds. The goal isn't to build bulky muscle, but to apply a specific type of therapeutic load to the tendons themselves. This controlled, high-tension lengthening is a unique stimulus that can't be replicated through traditional strength training or even running itself. The 'slow' component is critical; it ensures the tendon, not just the muscle, is bearing the load in a way that encourages adaptation rather than damage.
Retraining Tendons at a Cellular Level
So how does this actually work? Healthy tendons are made of strong, parallel collagen fibers. In an overused or injured tendon, these fibers can become disorganized and weak. Slow-eccentric loading has been shown to stimulate a process called mechanotransduction. This is where the mechanical force of the exercise signals the cells within the tendon (tenocytes) to get to work. These cells begin to produce new, healthy Type I collagen, the primary building block of strong tendons. Over time, this process helps to remodel the tendon, realigning the collagen fibers into a more robust and organized structure that is better equipped to handle the high-impact forces of running, especially the jarring demands of downhills.
Key Exercises for Hill Runners
Integrating slow-eccentrics into your routine doesn't require a lot of time or fancy equipment. The focus is on quality and control. Here are two foundational exercises: 1. Eccentric Heel Drops (for Achilles Tendon): Stand with the balls of your feet on the edge of a step. Rise up onto your toes using both feet (the concentric phase). Then, lift one foot off the step and very slowly lower the other heel down below the level of the step, taking at least 3-4 seconds. Return to the starting position using both feet and repeat. The Alfredson protocol, a well-studied method, suggests performing these with both a straight knee and a bent knee to target different parts of the calf and Achilles complex. 2. Slow Decline Squats (for Patellar Tendon): If you don't have a decline board, you can place a small weight plate under your heel to elevate it. Stand on one leg and perform a slow, controlled squat, again taking 3-4 seconds for the downward phase. Go as low as you can without pain, then use both legs to push back up to the starting position. This removes the concentric work from the affected leg, focusing purely on the eccentric load.
Integrating Eccentrics into Your Training
Consistency is more important than intensity. Start by incorporating these exercises two to three times per week on non-running days or after an easy run. Aim for 3 sets of 15 repetitions for each exercise. A key principle when dealing with tendinopathies is that some discomfort during the exercise is often acceptable, but it should not cause lingering pain the next day. If you experience increased pain 24 hours later, it's a sign to reduce the load or repetitions. Think of this as physical therapy for your tendons—a long-term investment in your running health that pays dividends on every hill you tackle.
