The Challenge of the Downhill
Runners love to conquer hills for the fitness benefits, but what goes up must come down. The downhill portion of a run, often seen as the reward, is actually where the knees face their greatest challenge. This is especially true for the patellar tendon,
which connects your kneecap to your shinbone. Each downhill stride requires your quadriceps to act as brakes to control your descent and absorb impact. This braking action is a specific type of muscle work that, when repeated over thousands of steps, can lead to overuse, micro-damage, and the nagging pain known as patellar tendinopathy, or 'runner's knee'.
Meet Eccentric Contractions
To understand the solution, we first need to understand the movement. Every exercise has two main phases. The concentric phase is when the muscle shortens to create movement, like when you push up from a squat. The eccentric phase is when the muscle lengthens while under tension, like the slow, controlled movement as you lower yourself into that squat. Think of it as 'negative work'—you are actively resisting a force, whether it's gravity or a weight. This lengthening-under-load action is precisely what your quads and patellar tendon do when you run downhill.
How Eccentric Work Strengthens Tendons
While it might seem counterintuitive to train the very motion that can cause pain, the magic is in the application. Performing slow, controlled eccentric exercises triggers a powerful adaptive response in your tendons. This specific type of loading stimulates the tendon's cells to produce more collagen, which is the primary building block of healthy connective tissue. This process helps to reorganise what might be disorganised fibres in an irritated tendon and, crucially, increases the tendon's overall strength and capacity to handle load. A stronger, more robust tendon is better equipped to absorb and manage the forces of high-volume running without breaking down.
Targeted Exercises for Patellar Protection
The most effective eccentric exercises for the patellar tendon are often simple and require minimal equipment. The goal is to isolate the tendon and load it slowly. One of the most evidence-backed exercises is the single-leg squat performed on a 25-degree decline board. This angle helps to maximize the load on the patellar tendon while minimizing strain on other parts of the leg. To perform it: Stand on one leg on the decline board. Use your other leg for balance. Slowly, over about 3-4 seconds, bend your standing leg into a squat, stopping before you feel sharp pain. Then, use your non-working leg to help you return to the starting position quickly. The focus is entirely on the slow, controlled lowering phase. Another effective exercise is a simple step-down. Stand on a step or sturdy box. Keeping your hips level, slowly lower the heel of your non-working leg toward the floor, taking several seconds to do so. Lightly tap the floor, then use your non-working leg to help push back up to the starting position.
Integrating Eccentrics Into Your Routine
Consistency is key to seeing benefits. Aim to perform these exercises two to three times per week on non-running days or after an easy run. The classic protocol often involves 3 sets of 15 repetitions, performed twice a day, but it is essential to start conservatively and listen to your body. Some discomfort in the tendon during the exercise is often acceptable and even expected, but sharp pain is a signal to stop or reduce the range of motion. Unaccustomed eccentric work can cause delayed onset muscle soreness (DOMS), so it is wise to start with just your body weight and gradually progress. Think of this not as a strenuous workout, but as targeted physical therapy to build more resilient tendons.
