New Delhi: Stroke is no longer just a disease of old age. New data from the Indian Council of Medical Research (ICMR) shows that around 13.8 per cent of stroke
cases in India occur in people between 18 and 44 years, meaning roughly 1 in every 7 stroke patients is under 45. The analysis is based on over 34,000 stroke cases recorded across 30 hospitals from January 2020 to December 2022. Stroke has now become the fourth leading cause of death and a major cause of disability in India, with young, working‑age adults increasingly affected.
This shift is worrying because many young Indians still think of stroke as a “senior citizen’s problem” and ignore early warning signs. The ICMR data also shows that nearly two in five stroke patients reach hospitals more than 24 hours after symptoms begin, which greatly reduces their chances of full recovery. 1 in 7 Indian stroke patients between ages 18-44: ICMR Study
The Indian Council of Medical Research led a large national stroke registry that collected information from 30 hospitals across India over three years. The study published in the International Journal of Stroke found that about 13.8 per cent of stroke patients were in the 18–44 age group, which translates to 1 in 7 stroke cases. Most of the remaining cases were in the 45–64 age group, while the number of very young stroke patients (below 18) was much smaller.
The data also showed that stroke is more common among men and in rural areas, which may reflect differences in healthcare access, lifestyle, and risk‑factor screening. Ischaemic stroke (caused by blocked blood vessels in the brain) made up about 60 per cent of all cases, while haemorrhagic strokes (caused by bleeding in the brain) were less common but usually more severe. Women accounted for roughly 36 per cent of the total cases, which is still a significant share of the burden.
Why strokes are happening earlier in India
Experts say strokes in India are occurring at a younger age compared with many Western countries. This is partly because of rising rates of high blood pressure, diabetes, and obesity, even in people in their 30s and 40s. Stress from work and city life, poor sleep, smoking, and high‑salt diets also increase the risk of stroke from a younger age.
Another major problem is that many young adults do not get regular health check‑ups and may not know they have high blood pressure or diabetes. By the time a stroke hits, damage to the arteries in the brain has already built up over years. Sedentary desk jobs, long hours on screens, and lack of physical activity further push the risk up, especially in urban populations.
Delayed treatment: Big problem
The ICMR analysis shows that only about 20 per cent of stroke patients reach hospital within 4.5 hours of symptom onset, which is the window when clot‑busting injections can be most effective. Around 38 per cent turn up after 24 hours, often because they ignore the warning signs or do not recognise them as a stroke. This delay means more brain cells die, leading to permanent disability or death in many cases.
In many Indian hospitals, advanced treatments such as thrombolysis and mechanical thrombectomy are still underused, with only about 4–5 per cent and 0.7 per cent of eligible patients receiving them, respectively. At the three‑month mark, nearly 28 per cent of stroke patients in the registry had died, and close to 30 per cent had major disability. These figures highlight how gaps in emergency care, transport, and awareness are worsening outcomes.
Common symptoms of stroke in young adults
Stroke symptoms usually appear suddenly and can affect anyone, even fit‑looking young people. The most common signs are sudden weakness or numbness in the face, arm or leg, usually on one side of the body. Trouble speaking clearly, difficulty understanding speech, sudden vision problems in one or both eyes, and sudden loss of balance or dizziness are also key warning signs.
A sudden, severe headache with no clear reason can also be a sign of stroke, especially if it comes with vomiting or confusion. If anyone around you shows these symptoms, even for a few minutes, it should be treated as an emergency. The “FAST” rule is easy to remember: Face drooping, Arm weakness, Speech problems, and the need to get to hospital Immediately (Time).
Main risk factors for young Indians
The biggest single risk factor for stroke is high blood pressure, followed by diabetes, high cholesterol, and heart disease. Smoking, heavy alcohol use, and a diet high in salt, fried foods and processed snacks also significantly increase the risk. Obesity and lack of physical activity make it harder for the heart and blood vessels to work well, raising the chances of a stroke.
Sleep problems such as sleep apnoea and chronic poor sleep are now being linked to more strokes in young adults. Stress‑related conditions, including anxiety and depression, can also indirectly raise blood pressure and weaken blood vessels over time. Young people who ignore recurrent headaches, dizziness or chest discomfort often miss an opportunity to catch these risk factors early.
How to protect yourself and your family
The good news is that stroke is largely preventable through lifestyle changes and proper medical care. Regular health check‑ups, especially for blood pressure, blood sugar and cholesterol, can detect problems before they lead to a stroke. Everyone over 30, and even some in their 20s with a family history of heart disease or stroke, should consider routine screening.
Eating a balanced Indian‑style diet with plenty of vegetables, whole grains, fruits and low‑fat dairy, and cutting down on salt, sugary drinks and deep‑fried snacks, can cut stroke risk. Walking for at least 30 minutes a day, using stairs, cycling, or doing yoga can help keep blood pressure and weight under control. Avoiding smoking, limiting alcohol, and getting at least 7–8 hours of good sleep are also important steps.
Building a stroke‑aware community
Schools, workplaces and housing societies can play a big role in spreading awareness about stroke. Simple campaigns that explain the FAST rule and what to do in an emergency can help people act quickly instead of waiting at home. Encouraging employees and students to get health check‑ups and offering stress‑management sessions can also reduce long‑term risk.
Families should also talk openly about stroke with parents, spouses and children, so no one ignores warning signs because they think “only old people get strokes.” Quick ambulance use, calling the nearest stroke‑ready hospital, and not relying only on home remedies can make a huge difference to recovery.
The ICMR study showing that 1 in 7 stroke patients in India is between 18 and 44 is a wake‑up call for all of us. Strokes are no longer rare in young adults, but many cases can be prevented with better awareness, healthier lifestyles, and quicker emergency care. By learning the warning signs, controlling risk factors and insisting on timely treatment, we can protect ourselves, our families, and future generations from this silent but deadly threat.
