New Delhi: Liver disease has quietly become one of the biggest health risks for people with diabetes. Recent Lancet‑linked study says it should be treated
as the “fourth major complication” of diabetes, alongside heart, kidney and nerve problems. In India alone, around one in four adults with type 2 diabetes already shows signs of serious liver scarring (fibrosis), and about one in 20 may already be on the road to cirrhosis, even though most feel perfectly fine with no obvious symptoms. This “silent” damage is why doctors are urgently calling for regular liver checks in every person with diabetes.
The new DiaFib‑Liver Study, published in The Lancet Regional Health – Southeast Asia, is the largest real‑world survey of its kind done in a low‑ or middle‑income country and involved more than 9,000 diabetes patients across 27 hospitals and clinics in India. It found that clinically significant liver fibrosis is now as common as kidney or eye problems in many people with diabetes, which is why experts are pushing for liver disease to be officially recognised as a “fourth major complication” of diabetes in national and global guidelines. For people living with diabetes, this means that checking the liver can no longer be an afterthought.
Key findings of the study
The DiaFib‑Liver Study looked at 9,202 adults with type 2 diabetes across 25 cities in India and used a painless liver‑stiffness test (FibroScan) to check how much scarring there was in their livers. About 26 per cent of patients showed clinically significant liver fibrosis, 14 per cent had advanced fibrosis, and roughly 5 per cent already met the threshold for probable cirrhosis.
Most of these people did not feel unwell, did not have jaundice or obvious liver symptoms, and were not being treated for liver disease, which shows how easily this damage can go unnoticed. Researchers say that, globally, diabetes is now one of the main drivers of advanced liver disease, especially non‑alcoholic fatty liver disease (NAFLD) and its more serious form, non‑alcoholic steatohepatitis (NASH).
Why liver disease is called the “fourth complication” of diabetes
For many years, doctors have mainly warned diabetics about three big risks: heart disease, kidney damage and eye or nerve problems. Now, because of the high number of patients with liver scarring and early cirrhosis, experts are saying that advanced liver disease should sit alongside these three as the “fourth major complication” of diabetes.
This change is not just a label; it is meant to nudge doctors and health systems to start including liver tests in routine diabetes check‑ups. The idea is simple: if kidney and eye screenings are standard, liver screening should eventually become standard too, especially in countries like India where diabetes and obesity are rising fast.
How common is fatty liver and liver damage in India?
Recent data from Indian studies show that non‑alcoholic fatty liver disease (NAFLD) is already very common and is closely linked to diabetes, obesity and metabolic syndrome. National‑level reviews suggest that around one in three adults or children in India may have NAFLD, and among people with type 2 diabetes, rates can go as high as 50–70 per cent.
In urban and semi‑urban Indian centres, studies of diabetic patients have reported NAFLD prevalence between about 45 per cent and 68 per cent, depending on how well blood sugar and weight are controlled. Government and public‑health reports also point out that as diabetes, obesity and metabolic syndrome become more common, the burden of fatty liver and liver disease is expected to increase sharply in the coming years.
How diabetes harms the liver quietly
When blood sugar stays high for a long time, extra fat tends to build up inside the liver, even if a person does not drink much alcohol. This causes NAFLD, which can then slowly progress from simple fatty liver to inflammation (steatohepatitis) and finally to scarring (fibrosis) and cirrhosis.
Several factors make this worse, including being overweight or obese, having high blood pressure or abnormal cholesterol, and having poor glycemic control (HbA1c above 7–9 per cent). Because the liver is very good at working even when partly damaged, many people can go for years without symptoms until the damage becomes serious and harder to reverse.
Symptoms to watch out for
In the early stages, liver disease linked to diabetes often has no clear signs, which is why it is called “silent.” As fibrosis gets worse or cirrhosis starts, a person may notice some of the following:

Health‑policy experts and specialists are urging that liver screening and management be added to national diabetes‑care programmes so that every person with diabetes receives at least a basic liver assessment.
