The Tennessee Department of Health issued low inspection scores to four restaurants in Robertson and Sumner counties in August for violations including flies in the kitchen and equipment covered in grease.
The Health Department gives restaurants with low scores a chance to correct issues, sometimes performing a follow-up inspection the same day. Follow-up inspection scores are provided when available.

Nashville Hot Chicken & Gyro
Score: 78
Date of inspection: Aug. 26
Examples of violations: Inspectors observed multiple repeat violations. Flies were seen in the kitchen. Cardboard that was deteriorating and covered in grease was being
used as a shelf liner. Food debris and grease build up was observed under kitchen equipment. Inspectors noted an employee beverage was stored above ready-to-eat foods in the cooler. The most recent inspection report was also not posted in the restaurant.

Jose's Mexican Restaurant
Score: 68
Date of inspection: Aug. 19
Examples of violations: Inspectors observed active managerial control over foodborne illness risk factors not being maintained. Health officials noted several flies in the kitchen, a cup with no handle stored directly in salsa, and ceiling tiles were observed building dust. A condenser drip in the walk-in cooler led to pooling water on the lids of food products and floor, causing visible floor damage by the dish pit.
Raw beef was found stored directly over cooked chicken, whole, uncut pineapples and milk jugs on wired shelving in the walk-in cooler. About 1 pound of chicken from a prior order being held for a potential, future order and 10 pounds of shrimp stored in grill drawers were embargoed during inspection due to elevated temperatures. About 65 pounds of refried beans that were not fresh were embargoed.

Alteno Mexican Restaurant
Score: 61
Date of inspection: Aug. 17
Examples of violations: No employees were seen washing their hands during inspection, health officials said in their report. Inspectors also found a pot of coffee along the waitress station and heavy mold buildup and ice accumulation in the reach-in freezers, including on stored food products.
About 25 pounds of salsa and carnitas were embargoed due to exceeding their 7-day hold. About 5 pounds of tilapia was also embargoed due to improper thawing. Erosion was observed due to prolonged water exposure from pooled water on floors near the dish pit. Intermittent-to-heavy grime buildup was seen on the floors, walls, doors and fixtures. Cardboard was found lining the floors of the dry stock, walk-in cooler and main cook line.
Alteno Mexican Restaurant scored a 78 on Feb. 9, and a 97 in a follow-up inspection on Feb. 19.
