Nearly 1 million Americans will begin receiving $500 refund checks tied to Affordable Care Act marketplace coverage under a new Trump administration initiative launching Sept. 30.
But for most Californians, the announcement comes with a catch: The state operates its own ACA marketplace, Covered California, which generally makes residents ineligible for the payments.
The administration says the refunds represent excess HealthCare.gov user fees that were built into health insurance premiums paid by some consumers using the federal exchange. The Treasury Department is mailing checks and letters signed by President Donald Trump to more than 950,000 qualifying Americans in 30 states.
Why most Californians won’t receive a $500 ACA refund check
California is one of 20 states that runs its own health insurance
marketplace rather than relying on HealthCare.gov, the federally operated exchange.
According to the White House, only consumers in states that use HealthCare.gov are eligible for the refund program. Because Californians buy marketplace plans through Covered California, the federal government did not collect the exchange user fees now being returned.
That means most California residents who buy health insurance through Covered California should not expect to receive a check, regardless of income level or whether they receive premium subsidies.
Who qualifies for the payments?
The administration says recipients must meet several requirements:
- Live in one of the 30 states that use HealthCare.gov.
- Have purchased ACA marketplace coverage through the federal exchange.
- Have paid the full cost of their 2026 health plan without receiving premium tax credits or other ACA subsidies.
Eligible consumers do not need to apply. The White House says it has already identified qualifying recipients and will mail checks automatically.
The payment is $500 per eligible enrollee, meaning households with multiple qualifying family members could receive more than one check.
What Covered California customers should know
For Californians, the announcement is largely a reminder that Covered California operates independently from the federal HealthCare.gov system.
As a result, most residents who buy health insurance through Covered California are not eligible for the $500 refund program.
Consumers with questions about their coverage or eligibility can review their marketplace information through Covered California, but state exchange enrollees generally should not expect to receive a payment under the federal initiative.
Which states are eligible?
The refund program applies only to residents in states that use HealthCare.gov: Alabama, Alaska, Arizona, Arkansas, Delaware, Florida, Hawaii, Indiana, Iowa, Kansas, Louisiana, Michigan, Mississippi, Missouri, Montana, Nebraska, New Hampshire, North Carolina, North Dakota, Ohio, Oklahoma, Oregon, South Carolina, South Dakota, Tennessee, Texas, Utah, West Virginia, Wisconsin and Wyoming.
California is not on that list because it operates Covered California as its own state exchange.
When will the checks arrive?
Treasury Department checks are scheduled to begin going out Sept. 30, with recipients expected to receive them in October.
People who believe they may qualify are encouraged to watch for official correspondence and make sure their mailing information is current.
The administration estimates nearly 1 million Americans will receive payments. By comparison, more than 19 million people are enrolled in ACA marketplace coverage nationwide, meaning the vast majority of enrollees will not qualify.
