Inspector Notes: Observed no certified food safety manager in the facility. Observed front kitchen hand sink water reached maximum temperature of 82 degrees. Observed rear hand sink without paper towels. Observed a pan of coconut shrimp and ice cubes stored in the reach-in freezer without a cover. Observed the ice machine containing black organic matter on the sides while the soda gun containing pink slime. Observed multiple TCS and RTE food items, such as cooked potatoes (date-marked 7/16), cooked seasoned beef (date-marked 7/29), chicken taco (date-marked 8/5), anchovies (date-marked 5/26), and mozzarella cheese (use by 5/26/26) stored in the prep-top and reach-in coolers for longer than 7 days. Observed disposable pizza boxes stored on the floor. Observed no temperature measuring device for the high-temp dish machine. Observed plumbing removed from 3-compartment sink.

Inspector Notes: Observed no current CFSM certificate posted in public view. Observed build-up of black substance on back wall of ice machine. Observed sliced tomatoes and sliced cheese cold-holding above 41F in prep-top cooler. Observed food debris build-up on outside of sliding door of freezer and warming cabinets. Observed several leaks coming from ceiling in main kitchen.

Inspector Notes: Observed raw animal food (chicken) stored above containers of ready-to-eat foods, including salsa and prepared blends, in the reach-in cooler. Observed raw shrimp stored next to raw hot dogs at the grill preparation area in the reach-in cooler. Observed containers of raw chicken stored next to bulk shelled eggs at the grill preparation area in the reach-in cooler. Observed combined ready-to-eat potentially hazardous food held more than 24 hours not date marked according to earliest date of opening/preparation. Food items and TCS sauces in storage containers were not identified with the common name of the food item. Observed raw fish (skin-on with head), sliced beef, and other raw animal meats being wrapped in grocery bags in freezers. Observed employee's water bottles near and on top prep raw chicken in reach-in cooler.
