Inspector Notes: Observed pizza prep-cooler not capable of maintaining temperatures of 41F or below. All food product that had been stored in unit longer than one hour held temperatures of 46F-60F. Products included: mozzarella cheese, marinara, sausage, beef, deli mix, pepperoni, ham, and tomatoes. Observed employee food stored directly above facility food in the walk-in cooler. Food included drinks, bananas and other groceries.

Observed several TCS foods cold-holding above 41F on top of salad bar units. Observed that facility had a high heat dishwasher but no method to verify high heat sanitization temperatures.

Inspector Notes: Observed handwashing sink in ware-washing area with cold water unable to used due to a stripped screw. Observed rag stored inside hand washing sink. Observed container of raw chicken stored next to container of raw beef in walk-in cooler.

Inspector Notes: Observed the facility to be allowing access to the kitchen ware-washing and dry storage area to children. Observed the facility to have and display an expired (4/26) CFSM certificate. Observed TCS food items on the front line prep cooler cold holding above 41 F. Observed the posted inspection report within the facility to be from 2023 and not the most recent.

Inspector Notes: Observed two hand washing sinks not accessible for employee use at all times. Observed improper vertical separation of raw animal foods and ready-to-eat foods in walk in cooler (raw shell eggs stored above produce). Observed several containers of uncovered food in holding unit of reach in cooler.

Inspector Notes: Observed fryer cooking/dispensing area with no hand washing sink available. Observed both hand washing sinks for men and women's restrooms with temperatures of 78F on both cold/hot taps. Observed build-up on baffle inside ice maker machine. Observed cooked chicken under heat lamp hot held at temperatures below 135F. Observed facility has an outdoor tap leading to floor drain without a curb. Observed floor in walk-in freezer covered with frozen water from dripping condenser unit. Observed broken floor tiles and coving in kitchen area. Observed no light in walk-in cooler. Observed facility back door wide open with broken air curtain above door.
