Inspector Notes: Observed no paper towels or other types of hand drying provisions at the employee handwashing sink. Observed no name labels on multiple drink mixes, TCS sauces, and juices at the drink mix stations. Observed no name on various types of meat throughout different reach in coolers. Observed cooked shrimp sitting in undrained ice/water in reach-in cooler. Unpackaged food was stored in direct contact with undrained ice. Observed in-use utensil (for rice) stored in standing water less than 135 degrees Fahrenheit.

Inspector Notes: Observed dishwasher in the bar being used with an empty sanitizer chemical container. Upon testing, the sanitizer was not registering on the test strip. Observed multiple items in reach in cooler above 41F. (beef patties, beef steak, calamari, shrimp, salmon, crab cakes, etc.) Observed multiple items in walk in cooler above 41F. (Chipotle aioli, short ribs, pork, meatballs, sausage and hamburger patties).

Inspector Notes: Observed food debris, dish scraper and yellow rag inside the main kitchen hand sink at the start of inspection. Observed TCS (raw shrimp, scallops, raw beef, etc.) and ready to eat (cook pork, beef) items throughout the kitchen (reach-in and walk-in cooler) without a date marked on them and employees stated that food items had been held for proximately 1-2 days. Observed improper thawing methods employed, frozen shrimp were thawing on the drain board of the prep-sink. Observed a lack of internal thermometers in cold-holding units. Observed ventilation hood to be dripping and contained a heavy accumulation of build-up.

Inspector Notes: Observed large wiping cloth buckets stored in front of hand wash sink near kitchen entrance. Observed diced tomatoes, pico tomatoes, and guacamole cold-holding above 41F in top of prep-top cooler. Observed facility did not have most current inspection report displayed.
