What does the health passport do?
The Health Passport is a physical document linked to a digital health database that records a child's complete medical history from birth to 18 years of age. Although annual health screening information has been uploaded to the SH-RBSK digital portal for several years, parents often lacked easy access to these records. The Health Passport bridges this gap by providing families with a portable document that can be carried during doctor consultations, hospital visits, emergencies, or follow-up appointments. The passport is updated regularly, ensuring that healthcare providers always have access to a child's latest medical information.Who will receive the health passport?
The scheme covers all children aged 0 to 18 years across Gujarat, benefiting approximately 1.89 crore children. Health screenings will be conducted through 992 mobile health teams across various institutions, including anganwadis, government schools, and private schools. Madrasas, gurukuls, and special schools. Parents are not required to submit additional paperwork. Once a child's health details are entered into the SH-RBSK digital system during the screening, the Health Passport is issued immediately.What information does the health passport include?
