A Breakthrough in the Fight Against Skin Cancer
Pharmaceutical giants Moderna and Merck have announced a significant milestone in oncology: their personalised mRNA cancer vaccine has succeeded in a large, late-stage clinical trial. The study focused on patients with high-risk melanoma, the most dangerous
type of skin cancer, whose tumours had been surgically removed. The results showed that combining the vaccine, known as intismeran, with the immunotherapy drug Keytruda, significantly delayed the cancer's return and prevented it from spreading compared to using Keytruda alone. This is the first time a personalised mRNA therapy for cancer has passed a Phase 3 trial, the final step before companies can seek regulatory approval, heralding a potential new era for cancer treatment.
How a 'Bespoke' Vaccine Works
Unlike traditional vaccines that prevent infectious diseases, this new therapy is a treatment designed to be used after cancer has already been diagnosed. Its power lies in its personalisation. The process begins by taking a sample of a patient's surgically removed tumour and sequencing its DNA. Scientists and AI algorithms analyse this genetic code to identify unique mutations, or 'neoantigens', that are specific to that individual's cancer cells. This information is then used to create a custom mRNA vaccine. When injected, the mRNA instructs the patient's healthy cells to produce harmless copies of these cancer-specific markers. This trains the immune system to recognise the unique flags of the cancer cells, priming it to hunt down and destroy any that might remain in the body or reappear later.
Decoding the Trial Results
The Phase 3 trial, named INTerpath-001, involved over 1,100 patients with high-risk melanoma. While the companies have not yet released the full data, they confirmed the trial met its main goals of improving recurrence-free survival and distant metastasis-free survival. This success builds on earlier, highly promising results from a smaller Phase 2 study. In that trial, an update after three years of follow-up showed the vaccine combination reduced the risk of recurrence or death by 49% compared to Keytruda alone. It also showed a significant reduction in the risk of the cancer spreading to other organs. The treatment was found to be well-tolerated, with most side effects being minimal, such as fatigue and fever. These strong results have been hailed by experts as a landmark moment for immunotherapy.
The Future of Cancer Treatment
The success in melanoma is likely just the beginning. Experts believe the principles of personalised mRNA vaccines could be applied to a wide range of other solid tumours. Because the vaccine targets the unique mutations of a tumour, the technology is theoretically adaptable to many forms of cancer. Moderna and Merck are already running trials for the same therapeutic approach in other cancers, including non-small cell lung cancer, bladder cancer, and kidney cancer. This breakthrough moves cancer care further into the realm of precision medicine, shifting away from one-size-fits-all treatments towards therapies that are tailor-made for each individual patient.
