Indian adults have some form of dyslipidemia, which is an unhealthy balance of fats in the blood. This condition typically involves high levels of 'bad' cholesterol (LDL), high triglycerides (a type of fat used for energy), and, most commonly in Indians, low levels of 'good' cholesterol (HDL). What makes this finding particularly alarming is that these unhealthy lipid profiles are not confined to those who are overweight or obese. A significant portion of individuals who are considered to be of a healthy weight are also at risk, pointing to a more complex health narrative for the Indian population.
India’s 'Metabolically Obese' Reality
The most telling part of this new data is the discovery of a large group of people who are 'metabolically obese' despite having a normal body mass index (BMI). An ICMR study found that a staggering 43.3% of Indians with a BMI under the 'overweight' threshold of 25 are, in fact, metabolically unhealthy. This group is classified as Metabolically Obese Non-Obese (MONO). To be classified as MONO, an individual with a normal weight must have at least two other metabolic risk factors, such as high blood pressure, elevated blood glucose, high triglycerides, or low HDL cholesterol. This means that nearly half of the people who appear slim and healthy are walking around with the internal metabolic chaos typically associated with obesity, putting them at silent risk for chronic conditions.
The Anatomy of South Asian Risk
This phenomenon is often referred to as the 'South Asian Paradox' or 'phenotype'. Indians have a unique genetic and physiological predisposition to store fat differently. Rather than subcutaneous fat (the kind you can pinch), there is a tendency to accumulate visceral fat—a dangerous type of fat that wraps around abdominal organs like the liver and intestines. This hidden fat is metabolically active, releasing inflammatory substances that disrupt normal body processes. It drives up triglyceride levels and lowers protective HDL cholesterol, creating a specific lipid pattern that is particularly atherogenic, or plaque-promoting, in arteries. In fact, recent studies have confirmed that for Indians, abdominal obesity—measured by waist circumference—is a far stronger predictor of abnormal cholesterol and heart disease risk than BMI alone.
A Mix of Genes and Lifestyle
This heightened risk is not purely genetic. While South Asians are genetically predisposed to conditions like insulin resistance and lower HDL levels, modern lifestyles have turned this predisposition into a full-blown public health crisis. The typical modern Indian diet, often high in refined carbohydrates (like white rice, naan, and biscuits) and sugar, provides the liver with excess energy that it converts directly into triglycerides. This dietary pattern, combined with increasingly sedentary jobs and reduced physical activity, creates a perfect storm for metabolic dysfunction. Even if your diet contains very little fat, a high intake of refined carbs can cause triglyceride levels to soar, contributing to the unhealthy lipid profile seen even in slim individuals.
Rethinking What 'Healthy' Looks Like
The key takeaway from this wealth of data is that we need to urgently redefine what it means to be healthy in India. Relying on the bathroom scale or a 'normal' BMI can provide a false sense of security. The evidence strongly suggests that cholesterol and metabolic health are not just issues for the visibly overweight. This hidden risk among normal-weight individuals means that millions could be progressing towards cardiovascular disease without any outward signs. Experts now advocate for a shift in public health strategy, moving away from weight-focused screening towards universal lipid profiling from a younger age. Knowing your numbers—your HDL, LDL, and triglycerides—is the only way to truly understand your metabolic health and take proactive steps to protect your future.
