What is a True Fruit Allergy?
A true fruit allergy is an immune system response. Your body mistakenly identifies a protein in the fruit as a threat and overreacts by producing antibodies called Immunoglobulin E (IgE). This response is usually rapid, with symptoms appearing within
minutes to two hours of eating the fruit. Symptoms can range from mild to severe and include skin reactions like hives or swelling, an itchy mouth or throat, digestive issues such as vomiting, and in the worst cases, a life-threatening reaction called anaphylaxis, which can cause difficulty breathing. One common type of fruit allergy is Oral Allergy Syndrome (OAS), which often affects people who also have pollen allergies. In OAS, proteins in certain raw fruits and vegetables are so similar to pollen proteins that they confuse the immune system, leading to symptoms usually confined to the mouth and throat, like itching or swelling.
Understanding Digestive Sensitivity
Unlike an allergy, a digestive sensitivity or intolerance doesn't involve the immune system. Instead, it happens in your digestive tract when your body has trouble breaking down a particular food. With fruits, this is often due to fructose, the natural sugar they contain. Some people have difficulty absorbing fructose, which can lead to bloating, gas, stomach cramps, and diarrhoea. This is a key feature of what are known as high-FODMAP foods (Fermentable Oligosaccharides, Disaccharides, Monosaccharides, and Polyols). Fruits like apples, mangoes, pears, and watermelon are high in FODMAPs and can trigger symptoms in people with conditions like Irritable Bowel Syndrome (IBS). Symptoms of intolerance can be delayed, sometimes appearing up to 24 hours later, and are often dose-dependent—meaning you might tolerate a small amount of the fruit but react to a larger portion.
The Role of Acidity and Reflux
Sometimes, the problem isn't an allergy or an intolerance but a case of acid reflux, or Gastroesophageal Reflux Disease (GERD). This occurs when the valve between your stomach and oesophagus weakens, allowing stomach acid to flow back up. Certain fruits, especially highly acidic ones like oranges, lemons, grapefruit, and even tomatoes, can worsen this condition. The acid in these fruits doesn't cause the reflux itself, but it can irritate the already sensitive lining of the oesophagus, triggering the classic symptoms of heartburn—a burning sensation in the chest—a sour taste in the mouth, or a persistent cough. It’s a mechanical and chemical irritation issue, completely separate from an immune or digestive breakdown problem.
How to Tell the Difference
Pinpointing the cause of your discomfort requires paying close attention to your symptoms. A true allergy often involves rapid, multi-system symptoms that can include skin and respiratory issues. An itchy mouth right after eating a raw apple is a classic sign of Oral Allergy Syndrome. Digestive sensitivity, on the other hand, is usually centred in the gut, with symptoms like bloating and gas that may appear hours later. If your main complaint is a burning in your chest after a glass of orange juice, acid reflux is a more likely culprit. Keeping a detailed food diary where you log what you ate, how much, and what symptoms you experienced (and when) can be an invaluable tool for you and your doctor.
Getting the Right Diagnosis
Self-diagnosing can be risky, especially since the symptoms can overlap and a true allergy can be dangerous. If you suspect a food allergy, it's essential to see a doctor or an allergist. They can perform tests like a skin-prick test or blood tests to measure IgE antibodies to confirm an allergy. For intolerances, a doctor might suggest an elimination diet, where you avoid suspected trigger foods and then reintroduce them to see if symptoms return. An oral food challenge, conducted in a medical setting, is considered the gold standard for diagnosing a food allergy definitively.
