The Old Assumption: Weight as a Warning
For years, the public health message has been straightforward: watch your weight to protect your heart. Body Mass Index (BMI), a simple measure of weight relative to height, became the go-to metric for assessing health risks. The logic seemed simple and
sound—higher weight often correlates with higher levels of LDL ('bad' cholesterol) and triglycerides, and lower levels of HDL ('good' cholesterol), a combination that significantly increases the risk of cardiovascular disease. Doctors, dietitians, and public awareness campaigns have all reinforced this idea, leading many to believe that maintaining a 'normal' BMI was a reliable shield against cholesterol problems. While it's true that obesity is a major risk factor for a host of metabolic issues, this focus on size alone has inadvertently created a dangerous blind spot, especially within the Indian context.
What the ICMR Study Really Found
The ICMR-INDIAB study, one of the largest and most comprehensive surveys of its kind, has painted a startlingly different picture. It revealed that an astonishing 87.3% of Indian adults have dyslipidemia, which means they have at least one abnormal lipid level in their blood. The most common issue wasn't just high 'bad' cholesterol, but alarmingly low levels of HDL, or 'good' cholesterol, affecting about two-thirds of the population. Here is the critical twist: these issues are not confined to those who are overweight or obese. The study found that dyslipidemia was present in 40% of individuals who had normal body weight, normal blood pressure, and no diabetes. This finding directly complicates the long-held belief that size is the primary determinant of cholesterol risk. It suggests that millions of Indians who appear healthy on the outside could be silently harbouring lipid abnormalities that put them at high risk for heart attacks and strokes.
The 'Thin-Fat Indian' Phenotype
So, how can someone be a normal weight but have unhealthy cholesterol? The answer may lie in a concept known as the 'thin-fat Indian phenotype', or normal weight obesity. This term describes a unique body composition common among South Asians, where an individual has a normal BMI but a high percentage of body fat, particularly visceral fat. Visceral fat is the dangerous, metabolically active fat that wraps around internal organs like the liver and intestines. Unlike the more visible subcutaneous fat, it releases inflammatory substances and is strongly linked to insulin resistance and abnormal cholesterol levels, such as high triglycerides and low HDL. This means that even if a person looks thin, their internal fat storage can be driving the same disease processes seen in someone with visible obesity. This phenotype is thought to be a result of a combination of genetic predisposition and environmental factors.
Why BMI Isn't the Full Story
The ICMR findings underscore a growing consensus among experts: BMI is an imperfect and often misleading metric for assessing health in the Indian population. Because of the 'thin-fat' phenotype, an Indian individual can have a 'healthy' BMI (below 25) but still carry enough visceral fat to be at high metabolic risk. This is why many experts now advocate for using other measurements alongside BMI, with waist circumference being a particularly important one. A larger waist circumference is a better indicator of abdominal obesity and the presence of harmful visceral fat. The study serves as a crucial reminder that health is more than a number on a weighing scale. Focusing solely on BMI can miss a huge segment of the at-risk population, delaying diagnosis and prevention until it is too late. The study's authors argue for a shift towards broader, universal screening for lipid abnormalities, rather than only testing those considered 'at-risk' based on weight.
Genetics, Diet, and What This Means for You
Beyond body composition, the widespread dyslipidemia in India is also influenced by genetics and modern lifestyles. South Asians are known to have a genetic predisposition for lower levels of good HDL cholesterol. This genetic tendency is now being amplified by significant lifestyle shifts. Diets are increasingly heavy in refined carbohydrates and unhealthy fats, which are known to raise triglycerides and lower HDL. At the same time, physical activity levels are declining across both urban and rural areas. The combination of these factors creates a perfect storm for poor heart health, regardless of body weight. The ICMR findings are not a reason to dismiss the importance of maintaining a healthy weight. Rather, they are a call to action to look beyond weight. The key takeaway is that no one should assume they are safe from cardiovascular risk based on their size alone. Regular health check-ups that include a lipid profile are essential for everyone.
