The BMI Blind Spot
For decades, Body Mass Index (BMI) has been the go-to metric for assessing healthy weight. It’s a simple calculation based on height and weight. However, this one-size-fits-all tool has a significant blind spot when it comes to the Indian population.
Health experts now widely agree that standard BMI cut-offs, developed primarily for Western populations, underestimate the health risks for people of South Asian descent. Indians tend to have a different body composition—less muscle mass and more body fat for the same BMI compared to people of European ancestry. This means an Indian with a 'normal' BMI might carry a level of body fat that would be considered unhealthy in other ethnic groups, putting them at risk for metabolic diseases without any obvious signs.
The 'Thin-Fat' Indian Paradox
This unique body composition is often described as the 'thin-fat' phenotype, or normal weight obesity. It refers to individuals who appear lean on the outside but have a high percentage of body fat, particularly visceral fat. Unlike the fat just under the skin (subcutaneous fat), visceral fat wraps around internal organs like the liver and pancreas, and is metabolically active. It releases inflammatory substances and hormones that can disrupt insulin function, raise blood pressure, and lead to unhealthy cholesterol levels. This internal fat accumulation is a key reason why many South Asians develop conditions like type-2 diabetes and heart disease at a younger age and lower body weight than other populations. The predisposition to this phenotype may even begin before birth, influenced by maternal nutrition and genetics.
An Alarming National Trend
Recent large-scale studies in India have painted a startling picture. According to a landmark national study by the Indian Council of Medical Research (ICMR), a staggering nine out of ten Indian adults have at least one abnormal lipid (fat) level in their blood. This condition, known as dyslipidemia, is widespread across both urban and rural areas. The most common issue is low levels of HDL, the 'good' cholesterol that helps clear fat from the bloodstream. At the same time, many have high levels of triglycerides, a type of fat linked to high carbohydrate diets. Worryingly, these risks are emerging in young adulthood, with a peak prevalence between 30-39 years, and millions of people who appear healthy with normal blood sugar and blood pressure still show abnormal lipid profiles.
Rethinking Health Assessment
Given these risks, relying on appearance or BMI alone is dangerously inadequate. Experts now emphasize the importance of looking at other indicators. Measuring waist circumference is a simple, effective way to estimate abdominal fat. For Indians, the recommended healthy waistline is stricter: below 90 cm (35 inches) for men and 80 cm (31 inches) for women. A measurement above these thresholds is a strong red flag for excess visceral fat and increased cardiometabolic risk, even if your BMI is normal. Comprehensive blood tests that go beyond just 'total cholesterol' are also crucial. A full lipid profile measures LDL ('bad' cholesterol), HDL ('good' cholesterol), and triglycerides, providing a much clearer picture of cardiovascular risk.
A Proactive Path Forward
Understanding this unique risk profile is not about causing alarm, but about empowering action. The knowledge that Indians are genetically predisposed to storing visceral fat and developing metabolic issues earlier should prompt a proactive approach to health. This involves shifting focus from just weight loss to improving overall body composition. Regular physical activity, including strength training to build muscle mass, is vital. Dietary changes are also key, particularly reducing the intake of refined carbohydrates and unhealthy fats, which are known to increase triglycerides and lower good cholesterol. Prioritizing a balanced diet rich in fibre, protein, and healthy fats can help manage this risk effectively. The goal is to move beyond the scale and adopt sustainable habits for long-term heart health.
