The Landmark National Study
The ICMR-India Diabetes (INDIAB) study is one of the largest and most comprehensive surveys on diabetes and non-communicable diseases (NCDs) ever conducted in the country. Spanning 31 states and union territories, researchers surveyed over 113,000 individuals
to get a real-world picture of India's health. The goal was to understand the true prevalence of conditions like diabetes, hypertension, and obesity across diverse urban and rural populations. What they found challenges the very definition of a healthy body, particularly in the Indian context.
Beyond the Body Mass Index (BMI)
For decades, Body Mass Index (BMI) has been the go-to metric for assessing healthy weight. However, the ICMR study confirms what many experts have suspected: BMI is a blunt and often misleading tool for Indians. The study revealed a startling statistic: a massive 43.3% of the Indian population falls into a category called Metabolically Obese Non-Obese (MONO). These are individuals who have a 'normal' BMI but suffer from metabolic issues typically associated with obesity, such as high blood pressure, elevated blood sugar, and unhealthy cholesterol levels. This finding proves that you can look slim on the outside while being metabolically unhealthy on the inside.
The 'Thin-Fat' Indian Phenotype
The high prevalence of MONO is explained by a concept known as the 'Asian Indian Phenotype', or more colloquially, the 'thin-fat' body type. Research shows that compared to other ethnic groups, Indians have a genetic predisposition to store more fat, particularly visceral fat, at a lower body weight. Visceral fat is the dangerous, unseen fat that wraps around your internal organs like the liver, pancreas, and intestines. This is different from the subcutaneous fat that lies just under the skin. Even in individuals who are not visibly overweight, this internal fat accumulation significantly increases the risk for insulin resistance, type 2 diabetes, and heart disease. This is why Indians tend to develop these conditions at a younger age and lower BMI than their Western counterparts.
The Real Culprit: Abdominal Obesity
If BMI is not the whole story, what should we be looking at? The ICMR-INDIAB study provides a clear answer: abdominal obesity. The research documented that a staggering 351 million adults in India have abdominal obesity. The fat stored around the abdomen is a far more accurate predictor of metabolic disease than overall weight. This is because a larger waist is a direct indicator of higher visceral fat. In fact, the study found that those with the highest risk for type 2 diabetes and coronary artery disease were those who were both metabolically obese and had general obesity (a high BMI). However, even those with a normal BMI but high central obesity carried significant health risks.
Measure What Matters: Your Waistline
The findings underscore an urgent need to shift focus from the weighing scale to the measuring tape. According to updated consensus guidelines for the Indian population, waist circumference is a critical health marker. For men, a waist circumference of 90 cm (approx. 35.5 inches) or more is considered a sign of central obesity. For women, the threshold is 80 cm (approx. 31.5 inches). To measure correctly, stand up straight and place a measuring tape around your middle, just above your hipbones, at the level of your navel. Ensure the tape is snug but doesn't compress your skin, and measure after you breathe out. This simple measurement provides a much more meaningful insight into your metabolic health than just your weight.
