When you fly directly into a high-altitude city—anywhere above 2,500 metres—you subject your body to a sudden drop in oxygen pressure. Unlike a gradual road trip, a flight gives you zero time to adapt. This rapid change can lead to Acute Mountain Sickness
(AMS), a common but preventable condition. The body needs time to make physiological adjustments, like increasing breathing rate and producing more red blood cells, to cope with less available oxygen. Rushing this process by planning strenuous activities immediately upon arrival is a recipe for headaches, nausea, and fatigue that can derail your trip.
Your First 48 Hours: The Golden Rules of Acclimatization
The first two days at altitude are critical. The most important rule is to take it easy. Your itinerary for day one should be simple: rest. Avoid any strenuous activity, including long walks or climbing stairs quickly. Hydration is your best friend; aim to drink three to four litres of water daily to combat the dry air and increased fluid loss from faster breathing. It’s also wise to avoid alcohol, which accelerates dehydration and can worsen AMS symptoms. For meals, opt for light, high-carbohydrate foods over heavy, fatty meals, as digestion slows down at high altitudes.
A Tale of Two Itineraries: The Right vs. Wrong Approach
Imagine two travelers arriving in Leh (3,500 metres). The first, eager to explore, immediately books a high-pass tour for the next morning. They spend the first day feeling dizzy and wake up on day two with a severe headache and nausea, forcing them to cancel their plans. The second traveler follows a cautious schedule. Day one is spent resting at the hotel, drinking water and herbal tea. On day two, they take a short, slow walk around the local market. By day three, their body has begun to adapt, and they are ready to explore the stunning landscapes, feeling healthy and energetic. This patient approach is key to enjoying your trip safely.
Smart Scheduling Strategies
When planning your trip, build in at least one to two full days for acclimatization before scheduling any major treks or tours. This is especially important if you are flying in from sea level. A well-known mountaineering principle is "climb high, sleep low." If possible, you can apply this by taking day trips to higher elevations but returning to sleep at a lower altitude. For example, when visiting Cusco, Peru (3,399 metres), some travelers opt to spend their first night in the lower Sacred Valley (~2,800 metres) to give their bodies a gentler introduction to the altitude before moving up.
Recognising the Red Flags
It's vital to listen to your body. Mild AMS symptoms like a persistent headache, dizziness, fatigue, and loss of appetite are common and should be taken as a signal to rest, not ascend further. Do not ignore these signs or dismiss them as simple jet lag. If symptoms worsen or you experience severe shortness of breath even at rest, confusion, or a persistent cough, it could indicate a more serious condition like High Altitude Pulmonary Edema (HAPE) or High Altitude Cerebral Edema (HACE). These are medical emergencies that require immediate descent and professional medical help.
