Your Oesophagus: The First Line of Defence
Think of your oesophagus as a muscular tube, about 25 centimetres long, that connects your throat to your stomach. Its inner wall is lined with a delicate, multi-layered tissue called the squamous epithelium. This lining is designed to be resilient, but
it's not built to withstand extreme temperatures. Its primary job is to help transport food and liquid smoothly, acting as a barrier to protect the deeper layers of the oesophageal wall. This lining is constantly renewing itself, but its defences are easily overwhelmed by repeated encounters with extreme heat.
The Immediate Shock of a Scald
When a liquid at a scalding temperature—well above what is comfortably warm—hits this lining, the effect is immediate and traumatic. The heat causes acute thermal injury. Cells are instantly damaged or killed, much like a burn on your skin. This triggers an inflammatory response as your body rushes to deal with the damage. You may not feel more than a momentary flash of heat, but on a microscopic level, your oesophagus is in crisis mode. In some cases, this can lead to symptoms like pain when swallowing, chest pain, and the formation of blisters or ulcers on the oesophageal lining. Endoscopic examinations have revealed tell-tale signs of thermal injury, including a distinctive "candy-cane" appearance where red, inflamed tissue alternates with white pseudomembranes formed by the inflammatory response.
The Danger of a Daily Habit
A single instance of drinking something too hot might cause temporary discomfort and reversible damage. The real danger, however, lies in repeated thermal stress. If you habitually drink beverages at scalding temperatures, you are not giving the oesophageal lining enough time to heal properly. Instead, you subject it to a cycle of chronic injury and inflammation. The body's response to this constant damage is to increase the rate at which it replaces the injured cells. This rapid cell turnover is a defence mechanism, but it's a frantic and imperfect one.
From Chronic Inflammation to Cancer Risk
This state of chronic inflammation and accelerated cell regeneration is a dangerous combination. Each time a cell divides, there is a small chance of a genetic error or mutation occurring. When cell division is happening at a much faster rate than normal, the chances of these errors accumulating increase significantly. This is the primary mechanism through which habitually drinking very hot liquids is linked to a higher risk of developing oesophageal squamous cell carcinoma, a type of oesophageal cancer. The International Agency for Research on Cancer (IARC), a part of the World Health Organization, has classified drinking very hot beverages—above 65°C (149°F)—as “probably carcinogenic to humans”. It's crucial to understand that the risk comes from the temperature, not the drink itself.
How Hot Is 'Too Hot'?
Many popular beverages are served at temperatures far exceeding the safety threshold. Coffee and tea are often served at between 71°C and 85°C (160°F to 185°F). A severe scald burn can occur in just five seconds at 60°C (140°F). The IARC identifies the danger zone as being above 65°C. Studies have shown that people who consistently drink tea at 60°C or higher have a significantly increased risk of oesophageal cancer compared to those who drink it at warmer temperatures. The ideal drinking temperature, which balances satisfaction with safety, is estimated to be around 58°C (136°F). This is a temperature that still feels pleasantly hot but gives your body a fighting chance to avoid injury.
Protecting Your Oesophagus Is Simple
The good news is that reducing this risk doesn't require giving up your favourite hot drinks. It simply requires a little patience. Allowing your tea, coffee, or soup to cool for a few minutes before you start sipping can make all the difference. Adding a splash of cold milk or water is also an effective way to bring the temperature down to a safer level. Pay attention to your body's signals; if a drink feels uncomfortably hot on your lips or tongue, it is certainly too hot for the delicate lining of your oesophagus. Waiting a bit not only protects your health but can also allow you to better appreciate the flavours of your drink, which are often masked at excessively high temperatures.
