U.S. Rep. Jill Tokuda, alongside Representatives Tracey Mann, Sharice Davids, Jack Bergman, Don Davis, and Carol Miller, has introduced the Rural Emergency Hospital Designation Improvement Act, also known as REH 2.0. This bipartisan measure aims to strengthen
the Rural Emergency Hospital (REH) designation, which was established by Congress in 2020 to provide a financial lifeline to struggling rural hospitals. The original REH program offered enhanced Medicare reimbursement for emergency and outpatient services and a monthly facility payment in exchange for transitioning away from traditional inpatient care. However, the program has fallen short of its promise, with only about 40 out of 1,500 eligible hospitals converting due to barriers such as the loss of inpatient and other essential services, 340B drug discounts, costly facility requirements, and restrictive eligibility rules. REH 2.0 seeks to address these shortcomings by allowing REHs to maintain distinct inpatient psychiatric and obstetric units, providing dedicated funding for laboratory services, and clarifying state Medicaid payment flexibilities. Companion legislation has also been introduced in the Senate by U.S. Sens. Jerry Moran and Tina Smith.
