A randomized study that followed participants for over 20 years suggests that the type 2 diabetes drug metformin may reduce the risk of dementia. The research, which tracked individuals from a 1990s clinical trial called the Diabetes Prevention Program
(DPP), found that only 1.9 percent of those given metformin developed dementia, compared to 4.2 percent in the lifestyle intervention group and 3.5 percent in the placebo group. The metformin group also exhibited significantly better memory performance. José Luchsinger at Columbia University in New York, who led the follow-up, noted that this provides the best available evidence, short of a long-term randomized clinical trial, that metformin may decrease dementia risk. However, the findings are not conclusive, and the exact mechanism by which metformin might offer this protective effect remains unclear. Previous studies have shown conflicting results regarding metformin's impact on dementia risk, with some suggesting a potential increase due to vitamin B12 deficiency, while a 2022 meta-analysis indicated a lower risk.

José Luchsinger is currently conducting a randomized controlled trial called Metformin in Alzheimer’s Dementia Prevention, which is following participants for 18 months to assess if the drug reduces the risk of Alzheimer’s disease. Results from this trial are anticipated by the end of the year. Additionally, Miia Kivipelto at the Karolinska Institute is leading the MET-FINGER study, which aims to determine if lifestyle changes alone or in combination with metformin can reduce dementia risk, with results expected in 2028. These ongoing studies are crucial for clarifying the mechanisms of action and confirming the protective effects observed in the long-term follow-up. The outcomes of these trials could lead to new recommendations for metformin use beyond diabetes management, potentially impacting millions of individuals globally.
