What's Happening?
The University of Oklahoma has initiated a new study aimed at increasing prostate-specific antigen (PSA) screening rates among Black men in Oklahoma, a demographic that experiences prostate cancer mortality rates two to four times higher than other racial
groups. The study, funded by a five-year, $2.9 million grant from the National Institutes of Health, will test an interactive app called Connect2Care. This app provides culturally tailored education on prostate cancer, helps users understand their personal risk, connects them with medical professionals, and guides them through an at-home PSA blood test. The at-home test results are then analyzed in a certified laboratory, and the app facilitates access to follow-up care if needed. According to study principal investigator Adam Alexander, Ph.D., early detection is crucial, as the five-year survival rate for prostate cancer is 90-100% when detected early, compared to 33% in later stages. A previous pilot study showed that Black men using Connect2Care were more informed about prostate cancer and more likely to complete at-home PSA tests than those using a standard informational app.

This study addresses a significant health disparity in the U.S., where Black men face disproportionately high prostate cancer mortality rates due to lower screening rates and later diagnoses. The Connect2Care app offers an innovative solution to overcome common barriers to in-person screening, such as transportation issues, work and family commitments, cost, and lack of healthcare access. By providing an at-home testing option and integrating comprehensive support within a single digital platform, the initiative aims to democratize access to crucial early detection services. Furthermore, the app incorporates a unique feature allowing men to confidentially speak with prostate cancer survivors, which helps combat the stigma associated with discussing the disease and encourages screening. If successful, this model could be scaled to other communities and health conditions, significantly improving health outcomes for underserved populations and reducing healthcare inequities.
What's Next?
The clinical trial will enroll 684 Black men in Oklahoma, aged 55-69, who are not up to date with PSA screening. Participants will be randomly assigned to one of three groups: the Connect2Care app, the U.S. Preventive Services Task Force app, or usual care (a brochure). Researchers will monitor participants for 30 days to determine PSA screening completion and conduct follow-up surveys. A six-month check-in will assess whether screening was completed and if follow-up treatment was sought for elevated results. The ultimate goal, according to Dr. Alexander, is for the app to be integrated into medical facilities' regular workflows to reach men facing screening barriers. The research team believes this 'one-stop shop' approach will simplify screening and ensure a seamless transition to medical providers for further evaluation or treatment.

The Connect2Care study highlights the growing potential of digital health interventions to address complex public health challenges, particularly in areas of health equity. By leveraging technology to deliver culturally sensitive education and facilitate at-home testing, the project not only aims to increase screening rates but also empowers individuals to take a more active role in their health management. The inclusion of peer support from prostate cancer survivors within the app is a critical element, recognizing the psychological and social barriers that often impede healthcare seeking behaviors. This approach could serve as a blueprint for future interventions targeting other stigmatized or underserved health conditions, fostering a more patient-centered and accessible healthcare system. The study also underscores the importance of community-based participatory research in developing effective and acceptable health solutions.
