What's Happening?
The Peterson Health Technology Institute (PHTI) has released an independent health technology assessment evaluating virtual solutions for managing chronic kidney disease (CKD) in stages 3–5. The assessment reviewed eight prominent commercial companies,
including DaVita Integrated Kidney Care (IKC), Evergreen Nephrology, Healthmap Solutions, Interwell Health, Kidneylink, Monogram Health, Somatus, and Strive Health. PHTI's findings indicate that these virtual CKD management programs show no consistent evidence of slowing disease progression, as measured by the adoption of guideline-recommended medications (ACEi/ARBs, SGLT2 inhibitors) or the rate of kidney function decline (eGFR slope), when compared to standard care. The systematic literature review, which included 34 clinical studies and multi-year evaluation data from CMS’s Kidney Care Choices (KCC) model, found no significant alteration in annual eGFR decline slopes or reduction in progression toward end-stage kidney disease (ESKD). Furthermore, there were no meaningful differences in prescribing rates of kidney-protective therapies between patients enrolled in virtual programs and those receiving usual care. Five of the eight companies evaluated, despite significant private capital and market capitalization, produced no peer-reviewed clinical evidence meeting review standards.

The finding that only a small fraction of attributed CKD patients (0.7% to 1.9%) experience an improved transition to dialysis, despite significant investment, suggests that the current model may not be delivering broad population-level benefits. This could lead to a re-evaluation of how value-based care contracts are structured and how technology is integrated into chronic disease management.

PHTI recommends that purchasers and the Centers for Medicare & Medicaid Services (CMS) restructure kidney value-based contracts. Instead of relying on broad population benchmarks, the institute suggests shifting towards discrete patient-level milestone payments. These payments would be tied to specific outcomes such as early eGFR/uACR testing, verified initiation of generic kidney-protective drugs by primary care physicians, and utilization-based cost offsets. This proposed shift aims to incentivize interventions that demonstrably impact disease progression and reduce costs. The findings may also prompt the evaluated companies to invest more in robust, peer-reviewed clinical trials to substantiate the effectiveness of their virtual CKD management platforms. Additionally, there could be increased scrutiny from investors and healthcare payers regarding the return on investment for these types of digital health solutions, potentially leading to changes in funding and market strategies within the virtual care sector.

The PHTI assessment reveals deeper implications regarding the integration of technology into healthcare and the challenges of achieving true value-based care. The report suggests that the current shortfalls in virtual CKD management stem from misaligned payment incentives, rather than a lack of intent from clinicians. A significant issue identified is the 'primary care gap,' where approximately 87% of adults with CKD are unaware of their condition, and virtual vendors often engage patients only after a formal diagnosis, limiting early intervention. This highlights a systemic challenge in reaching patients at critical early stages of chronic diseases. Furthermore, the assessment points to a potential disconnect between 'coding' for risk documentation to boost revenue and actual cost reduction, particularly under Medicare Advantage's 'Medical Loss Ratio reduction guarantees.' This raises ethical questions about how financial incentives might inadvertently prioritize documentation over tangible patient outcomes and cost savings, underscoring the complexity of reforming healthcare payment models to genuinely improve patient health and reduce overall expenditures.
