SocialRx, a social prescribing platform, has partnered with Urban Health Plan, a federally qualified health center in New York City, to offer arts and culture prescriptions to patients with chronic illnesses. This initiative aims to improve mental health and reduce

The partnership targets patients aged 18 to 24 with obesity and depression, and those aged 55 to 70 with hypertension and depression. SocialRx provides personalized well-being plans and follows up with patients to assess the effectiveness of the engagements.
