A recent study published in *Nature* details the synergistic effects of combining 177Lu-DOTATATE peptide receptor radionuclide therapy (PRRT) with sunitinib in treating progressive pancreatic neuroendocrine tumors (NETs). Five patients with metastatic
pancreatic NET G2 or G3, who had shown disease progression under previous treatments like PRRT, chemotherapy, or everolimus, received this combination therapy. Patients were administered 1–3 cycles of 177Lu-DOTATATE (7.4 GBq per cycle) at 12-week intervals, with sunitinib (37.5 mg/d) given in between PRRT cycles. Follow-up 68Ga-DOTATOC PET imaging revealed a near-complete response in three patients and a partial response in two patients, a surprising outcome given their prior progression. The combination therapy was well-tolerated, with some patients requiring a sunitinib dose reduction to 25 mg/d. Laboratory parameters showed no significant reduction in creatinine and glomerular filtration rates, though platelet counts and hemoglobin values were reduced in some cases. In vitro studies confirmed that 177Lu-DOTATATE and sunitinib synergistically inhibit cell proliferation and induce cell death in QGP-1 pancreatic NET cells.
