What's Happening?
Telemedan, a telemedicine initiative, is expanding healthcare access in rural Chad through solar-powered kiosks. Launched in 2021 by Abakar Mahamat, the initiative aims to bring medical consultations closer to communities with limited electricity, internet
access, and medical infrastructure. For residents like Ache Kodo in Bitkine, these kiosks eliminate the need for costly and time-consuming travel to larger towns for medical care. Telemedan kiosks operate on solar panels and batteries, ensuring functionality without reliable grid electricity. Local operators assist patients by registering them and taking basic measurements using medical equipment such as blood-pressure monitors, glucometers, thermometers, pulse oximeters, and ECG machines. Doctors then conduct remote consultations via screen. The system is designed to function even with unreliable internet, storing patient information locally and synchronizing it when connectivity is restored. An Unstructured Supplementary Service Data (USSD) service allows individuals without internet to book appointments and access health information. The initiative has grown from a pilot project to 37 kiosks across 11 provinces, serving an estimated 143,000 people, with support from grants and partnerships, including Chad’s Ministry of Public Health and Prevention.

Furthermore, the integration of basic diagnostic tools and remote physician access significantly improves the quality and timeliness of care, potentially leading to earlier diagnosis and treatment of various conditions. This model also empowers local communities by training operators and leveraging technology to bridge critical service gaps, enhancing overall public health outcomes.
What's Next?
Telemedan plans to continue its expansion, with an ambitious goal of reaching one million people in rural communities across sub-Saharan Africa. The initiative is also exploring advanced technologies, such as using artificial intelligence to screen for diabetic retinopathy, aiming to assist healthcare professionals rather than replace them. Chad's government is actively supporting such digital health solutions; the National Health Development Plan for 2022–2030 identifies digital health as a key strategy to expand care access. The Ministry of Health is developing a legal and regulatory framework for digital health services, which will further integrate and legitimize initiatives like Telemedan. However, challenges remain, including ensuring the long-term maintenance of equipment, consistent connectivity, ongoing training for local operators, and establishing clear pathways for patients requiring care beyond what a kiosk can provide. The sustainability model involves selling kiosks to governments and partner organizations, which then operate them, ensuring patients do not bear the infrastructure cost.
