A collaborative study by the Mayo Clinic and Virginia Tech, published in Nature Communications, has identified five distinct subgroups of metabolic dysfunction-associated steatotic liver disease (MASLD), formerly known as non-alcoholic fatty liver disease.
Researchers analyzed clinical histories and genetic data from over 4,600 patients to uncover patterns that were previously obscured when MASLD was treated as a single condition. The study categorized these subgroups, labeled C1 through C5, each exhibiting different clinical characteristics and risks. For instance, C2 predominantly featured men with a cardiorenal profile, while C3 was more common in women and linked to obesity and mood disorders. Notably, C4, despite having fewer metabolic comorbidities, showed the highest rate of liver transplants, attributed to an unfavorable combination of genetic variants. C5, characterized as polygenic MASLD/MASH, presented a higher risk of advanced fibrosis and acute renal failure. This research suggests that MASLD is not a singular disease but a heterogeneous condition with varied underlying mechanisms and prognoses.
