A post hoc analysis of the Myocardial Ischemia and Transfusion (MINT) trial has examined the effects of liberal versus restrictive transfusion strategies in patients with myocardial infarction (MI) and anemia. The trial, conducted from April 2017 to April 2023,
involved 3,504 participants across 144 sites in six countries. Participants were randomized to either a restrictive transfusion strategy, where transfusions were given if hemoglobin levels fell below 7 to 8 g/dL, or a liberal strategy, maintaining hemoglobin levels above 10 g/dL. The analysis found that the liberal strategy was associated with a 1.4% to 2.4% lower risk of 30-day death or MI compared to the restrictive strategy. However, the liberal strategy also showed a slightly higher risk of heart failure, ranging from 0.2% to 0.6% more than the restrictive approach.
