What's Happening?
b.well Connected Health, a consumer health data integration platform, has partnered with MyDirectives, a leader in digital advance care planning (ACP), to integrate advance care planning documents directly into longitudinal medical records. This collaboration
marks a significant advancement in health data rights by allowing patients to access their critical end-of-life and acute medical preferences alongside their daily health information. Historically, these documents were often stored in inaccessible physical formats, leading to critical data failures during emergencies. The integration connects b.well’s FHIR-based data network and CMS Aligned Network with MyDirectives’ A|D Vault Exchange™, which houses the largest nationwide registry of advance directives and portable medical orders. This partnership is the first instance where MyDirectives has integrated with a consumer-facing digital health organization specifically for patient-directed access, moving beyond traditional clinical facilitator and first-responder lookup portals.
Why It's Important?
This partnership addresses a critical gap in the U.S. healthcare system where patient preferences for end-of-life and acute care are often unavailable to clinicians during emergencies. By digitizing and integrating advance directives into consumer health records, it ensures that patients' wishes are readily accessible, improving the quality and personalization of care, especially in crisis situations. This enhances patient autonomy and reduces the burden on families to communicate complex medical decisions under duress. For healthcare providers, it means more informed decision-making, potentially reducing medical errors and ethically challenging situations. The move also aligns with broader efforts to improve health data interoperability and patient access to their own health information, fostering a more patient-centric healthcare ecosystem and potentially reducing healthcare costs associated with unwanted or unnecessary treatments.
What's Next?
The integration will allow advance care planning documents to be ingested through b.well’s FHIR-based platform and presented as a native data type within the consumer’s unified health summary. This means that patients will have direct control and access to their advance directives, and these documents will be available to healthcare providers through secure, interoperable networks. The success of this model could encourage other digital health platforms and healthcare systems to adopt similar integrations, further expanding the reach and accessibility of advance care planning. This initiative is expected to improve emergency care outcomes, enhance patient satisfaction, and promote a more proactive approach to healthcare planning across the nation. Continued efforts will likely focus on increasing public awareness and adoption of digital advance care planning tools.
