What's Happening?
Pretzel Therapeutics, a clinical-stage biotechnology company, has received UK regulatory authorization to advance its lead therapeutic candidate, PX578, into clinical development for the treatment of POLG-mediated primary mitochondrial disorders. This
follows the U.S. Food and Drug Administration (FDA) clearance of the Investigational New Drug (IND) application for PX578 in August 2026 and the FDA's Fast Track Designation in September 2026. These regulatory milestones pave the way for the global clinical development of the Phase 2 POLARIS (POLg Activation and Recovery In Subjects) study, which will evaluate PX578 in adult patients with POLG disease. PX578 is designed to activate POLG and increase mitochondrial DNA, aiming to restore mitochondrial function and potentially modify the underlying disease process for this rare, progressive genetic disorder.
Why It's Important?
The FDA's clearance and Fast Track Designation for PX578 highlight the urgent unmet medical need for effective treatments for POLG-mediated primary mitochondrial disorders. Currently, there are no approved disease-modifying therapies specifically targeting the underlying cause of POLG disease, which can lead to debilitating pain, physical limitations, and early mortality. The advancement of PX578 into Phase 2 clinical trials offers a potential new therapeutic option for patients in the U.S. and globally. The Fast Track Designation is particularly significant as it is intended to facilitate the development and expedite the review of drugs that treat serious conditions and fill an unmet medical need, potentially bringing this treatment to patients sooner.
What's Next?
Pretzel Therapeutics is on track to initiate the Phase 2 POLARIS study by the end of 2026. This randomized, double-blind, placebo-controlled trial will assess the safety, tolerability, pharmacokinetics, pharmacodynamics, and clinical efficacy of PX578 in adults with POLG disease. Successful outcomes from this trial could lead to further regulatory steps and potentially accelerate the availability of PX578 as a disease-modifying treatment. The company's focus will be on executing the clinical development plan and gathering robust data to support future marketing authorization applications in both the U.S. and the UK.
