The ECSTATIC feasibility trial investigated whether children receiving extracorporeal membrane oxygenation (ECMO) can be safely managed with different prophylactic platelet transfusion thresholds. Conducted across nine centers in the United States and
one in Israel, the open-label, randomized trial enrolled 50 children younger than 18 years who had no or minimal bleeding within 24 hours of cannulation. The study compared two transfusion triggers: a lower threshold of less than 50 × 10^9/L and a higher threshold of less than 90 × 10^9/L. The trial successfully achieved a clear separation in pre-transfusion platelet counts between the two groups and demonstrated excellent protocol adherence. This outcome supports the feasibility of conducting a larger, definitive trial to determine the optimal platelet transfusion threshold for pediatric ECMO patients. The median age of participants was 0.2 years, with 44 out of 50 patients receiving venoarterial ECMO.
