New research indicates that continuous glucose monitors (CGMs), while transformative for diabetes care, have a significant limitation: they cannot accurately measure blood sugar levels during extreme high or low episodes. Instead, when glucose levels move
beyond their fixed measurement range, CGMs can only report that levels are extremely high or low, without specifying the exact severity. This blind spot was identified through an analysis of nearly 47 million CGM readings from 948 individuals with type 1 diabetes across four international clinical trials. The study found that between 93.5% and 100% of participants experienced at least one reading that hit the upper or lower measurement limit. For high glucose levels, over one-third of these episodes in three studies remained at the upper limit for at least an hour, with the longest lasting two to three hours for a typical participant. Younger individuals and those with higher HbA1c levels were more frequently affected by these limit readings.
