What's Changed?
In September 2024, the Indian government expanded Ayushman Bharat PM-JAY with a major update. All senior citizens aged 70 and above, regardless of income or economic status, are now eligible for ₹5 lakh in additional health coverage on top of the existing ₹5 lakh.
This additional ₹5 lakh is:
- Exclusively for the senior citizen (not shared with other family members under 70)
- Available even if the family already has private health insurance
- Cashless at any empanelled hospital across India
Who Is Eligible for PM-JAY?
Category 1: The Original Beneficiaries (₹5 Lakh Cover)
PM-JAY covers approximately 12 crore families (55 crore individuals) from economically weaker sections. Eligibility is based on the Socio-Economic Caste Census 2011 (SECC) data.
In rural areas, you are likely eligible if your family is:- Living in a kutcha house (one-room, one wall)
- Landless, earning through manual labour
- Headed by a woman with no adult male member
- Belonging to SC/ST households
- A family with a disabled member and no able-bodied adult
- Rag pickers, domestic workers, street vendors
- Construction workers, plumbers, painters
- Security guards, sweepers, sanitation workers
- Transport workers, rickshaw drivers, shop assistants
Category 2: Senior Citizens 70+ (Additional ₹5 Lakh)
This is the new expansion:
- Any Indian citizen aged 70 or above. No income criteria.
- Even if they are already covered under CGHS, ECHS, or private insurance
- They get a separate health card for this benefit
What PM-JAY Covers
- All hospitalisation expenses (room, bed, nursing)
- Doctor and specialist consultation fees
- Medicines and medical consumables
- Diagnostic tests (blood work, X-rays, MRI, etc.)
- Pre-hospitalisation expenses (up to 3 days before admission)
- Post-hospitalisation expenses (up to 15 days after discharge)
- All pre-existing diseases from day one
- Over 1,900 medical and surgical procedures
What PM-JAY Does Not Cover
- OPD (Out-Patient Department) consultations
- Cosmetic surgery
- Fertility treatments
- Drug rehabilitation
- Dental treatments (unless related to a covered procedure)

Method 3: Visit a Common Service Centre (CSC)

- Treatment is completely cashless — the hospital bills the government directly
- Works at 28,000+ empanelled hospitals across India (both government and private)
- No restriction on family size — all members listed under your SECC data are covered
- No cap on family members — unlike private insurance, there is no "individual sub-limit"

The Real Problem: Awareness
The scheme has been running since 2018. Over 27 crore Ayushman cards have been generated. Over 5 crore+ hospital admissions have happened under it. And yet, millions of eligible families do not know they qualify.
Reasons include:- Low digital literacy in rural areas
- Language barriers: portal is primarily in English and Hindi
- No proactive outreach in many states
- Private hospital staff sometimes do not inform patients about PM-JAY eligibility
- People assume they do not qualify without actually checking
